Abstract
Osteoartritis merupakan penyakit radang sendi yang disebabkan proses mekanik seperti trauma dan beban berat sehingga mengakibatkan kerusakan sendi kronis dan gangguan fungsi sendi. Sargassum sp, mengandung fucoidan yang mampu meningkatkan mobilisasi mesenchymal stem cell melalui peningkatan ekspresi CXCR-4, mencegah degradasi dengan penurunan kadar MMP-3 serta mencegah inflamasi dengan peningkatan kadar IL-1. Tujuan penelitian ini adalah untuk menguji potensi fucoidan dalam meningkatkan ekspresi CXCR-4 pada jaringan sendi tikus model osteoartritis serta pengaruhnya terhadap kadar MMP-3 pada serum darah tikus. Penelitian ini menggunakan tikus model osteoartritis yang dibagi menjadi sembilan kelompok. Kelompok kontrol positif dan perlakuan diinduksi osteoartritis dengan 3 kali injeksi CFA tiap 2 minggu. Kelompok A, B dan C diberi ekstrak fucoidan 20, 40 dan 80 mg/kgBB, perlakuan D diberi steroid 10 mg/kgBB, perlakuan E, F dan G diberi kombinasi steroid dan fucoidan sesuai dosis sebelumnya sebanyak 2 kali dengan interval 1 minggu. Derajat nyeri diukur setiap minggu. Tikus dibedah kemudian diambil sampel jaringan sendi untuk pemeriksaan ekspresi CXCR-4 dengan imunohistokimia dan kadar MMP-3 dalam serum. Pemberian terapi fucoidan mampu mengurangi nyeri sendi pada tikus model osteoartritis secara signifikan (p<0,05). Selain itu, pemberian fucoidan dapat menurunkan kadar serum matrix metalloproteinases-3 pada tikus model osteoartritis secara signifikan. Hasil pemeriksaan imunohistokimia juga menunjukkan peningkatan ekspresi CXCR-4 pada kelompok perlakuan dibanding kelompok kontrol. Respon terapi terhadap fucoidan menunjukkan hasil lebih baik dibandingkan penggunaan steroid injeksi yang biasa digunakan sebagai terapi pada osteoartritis. Dengan demikian, injeksi fucoidan secara intraartikular memiliki potensi sebagai alternatif terapi yang efektif untuk osteoartritis.
English Abstract
Osteoarthritis is an inflammatory disease of the joints caused by mechanical processes such as trauma and heavy loads, resulting in chronic joint damage and impaired joint function. Sargassum sp, containing fucoidan were able to increase the mobilization of mesenchymal stem cell through increased expression of CXCR-4, preventing degradation by a decrease in the levels of MMP-3 and prevent inflammation by elevated levels of IL-1. The purpose of this study was to test the potential of fucoidan in increased expression of CXCR-4 in the joint tissue of mice model of osteoarthritis and its influence on the levels of MMP-3 in the blood serum of mice. This study uses a rat model of osteoarthritis were divided into nine groups. Positive control group and the treatment of osteoarthritis induced by CFA injection three times every two weeks. Group A, B and C were given extracts of fucoidan 20, 40 and 80 mg / kg, given steroid treatment D 10 mg / kg, treatment E, F and G are given a combination of steroids and fucoidan corresponding previous dose 2 times with an interval of one week. The degree of pain was measured every week. Mice dissected later joint tissue samples taken for examination CXCR-4 expression by immunohistochemistry and MMP3 levels in serum. Fucoidan therapy can reduce joint pain in a rat model of osteoarthritis significantly (p <0.05). In addition, administration of fucoidan may decrease serum levels of matrix metalloproteinases-3 in the rat model of osteoarthritis significantly. The results of immunohistochemical examination also showed increased expression of CXCR-4 in the treatment group than the control group. The response to fucoidan therapy showed better results than the use of steroid injections used in the treatment of osteoarthritis. Thus, fucoidan as intraarticular injection has potential as an effective alternative therapy for osteoarthritis.
