Abstract

Prevalensi penyakit LES terus meningkat di Indonesia. Vitamin D diketahui dapat berpengaruh terhadap sistem imun dan dapat mencegah autoimun pada LES. Vitamin D sebagaian besar disintesis tubuh dengan bantuan paparan sinar matahari dan sedikitnya berasal dari asupan makanan. Penelitian ini bertujuan untuk mengetahui hubungan paparan sinar matahari dan asupan vitamin D terhadap status vitamin D pada pasien Lupus Eritematosus Sistemik. Penelitian menggunakan metode case control pada 28 pasien LES wanita di Poliklinik Rematologi Rumah Sakit Saiful Anwar Malang dengan usia 16-50 tahun dan 20 responden kontrol sehat. Serum vitamin D (25(OH)D3) dianalisa menggunakan metode ELISA (Cusabio). Data asupan vitamin D menggunakan kuisioner semi quantitative FFQ dan faktor-faktor yang mempengaruhi sintesa paparan sinar matahari diambil menggunakan kuisioner sun exposure. Perbedaan paparan sinar matahari antar kelompok dianalisa menggunakan oneway Anova test . Hubungan antara asupan vitamin D dengan kadar vitamin D diuji dengan Pearson test sedangkan hubungan paparan sinar matahari dengan kadar vitamin D diuji dengan Spearman test . Hasil penelitian menunjukkan kadar vitamin D pasien LES lebih rendah (23,89 ± 6,22 ng/ml) dibandingkan kontrol sehat (35,06 ± 6,43 ng/ml;p =0,000). Terdapat perbedaan yang signifikan antara asupan vitamin D kelompok LES dengan kelompok kontrol sehat (p=0,007). Pada pasien LES tidak terdapat hubungan yang signifikan antara asupan vitamin D (p= 0,55) dan paparan sinar matahari (p=0,758) terhadap status vitamin D. Kesimpulan dari penelitian ini adalah kadar vitamin D pada pasien LES lebih rendah dibandingkan kelompok kontrol. Hal ini mungkin disebabkan oleh asupan vitamin D serta paparan sinar matahari yang lebih rendah meskipun tidak terdapat hubungan yang signifikan serta faktor patologis pada pasien LES.

English Abstract

The prevalence of LES in indonesia continues to rise. Vitamin D is known that effect immune system and prevent autoimmunity. Most of vitamin D synthesis by sun exposure and a few from daily intake. The objective of our study was to prove the relation of sun exposure and intake of vitamin D with vitamin D status in patients with LES. This observational study used case control performed in 28 female SLE patients who were 16-50 years old and were visiting Rheumatology Division, Dr.Saiful Anwar Hospital Malang and 20 healthy controls. Serum vitamin D (25(OH)D3) level was assessed using ELISA (Cusabio)method. Data of intake vitamin D collected by semi quantitative FFQ questionnaire and factors affecting synthesis of sun exposure taken using sun exposure questionnaire. Meanwhile, differences exposure to sunlight between 2 groups analysis used oneway anova test.The correlation between intake of vitamin D and serum vitamin D (25(OH)D3) level was analysed with Pearson test. The correlation between sun exposure and status vitamin D level was analysed with Spearman test. The results showed that serum level of vitamin D in SLE patients was significantly lower 23,89 ± 6,22 ng/ml than in healthy controls 35,06 ± 6,43 ng/ml (p =0,000). There was a significant difference between vitamin D intake group LES with a healthy control group (p = 0.007). The results showed that there was no significant correlation between intake of vitamin D level (p=0,55) , sun exposure (p=0,758) and status vitamin D level. Conclusion of this study is vitamin D levels in patients LES lower than control group. This may be caused by the intake of vitamin D and sun exposure is lower even though there was no significant relationship and pathological factors in patients of LES.
