Abstract

Hiperglikemi merupakan keadaan dimana kadar gula darah ≥ 126 mg/dl merupakan penyulit penyembuhan luka. Kandungan Binahong berupa saponin, flavonoid, polifenol, triterpenoid, antosianin, asam ursolat dan karbonat diduga dapat membantu mempercepat proses penyembuhan luka dan mengurangi nekrosis. Kandungan CMC pada basis hidrogel meningkatkan ekspresi gen TNF-a yang dapat meningkatkan jumlah limfosit dan neutrofil pada sel endotel. Nekrosis merupakan jaringan mati yang ditemukan pada luka karena perubahan morfologi dimana terdapat 3 indikator nekrosis yaitu peningkatan jumlah Debris sel dan PMN (Polimorfonuklear) serta penurunan plasma sel. Tujuan penelitian ini adalah untuk mengetahui pengaruh perawatan luka hiperglikemi menggunakan hidrogel binahong terhadap penurunan nekrosis. Desain penelitian menggunakan true-experiment post-test dilakukan terhadap hewan coba tikus putih jantan galur wistar. Jumlah sampel adalah 30 tikus (n=5) dan dibagi dalam 6 kelompok yaitu 4 perlakuan hidrogel binahong konsentrasi 2,5%, 5%, 7,5%, kontrol (-) NS dan kontrol (+) menggunakan basis hidrogel. Data yang diukur adalah penurunan indikator nekrosis pasca perawatan luka selama 12 hari. Analisis Kruskal Walls didapat p = 0,007 (debris sel) dan uji One-Way ANOVA didapatkan p = 0,000 (plasma sel), p = 0,006 (PMN) (p < 0,05). Melalui uji Post Hoc Test indeks nekrosis dengan p = 0,002 (debris sel), p = 0,000 (plasma sel), p = 0,005 (PMN) (p < 0,05). Dapat disimpulkan bahwa perawatan luka menggunakan kombinasi hidrogel binahong mempengaruhi penurunan nekrosis jaringan luka hiperglikemi.

English Abstract

Hiperglycemia is a condition with high blood sugar (>126) mg/dl) in wound condition could difficult to healed. Binahong it consists of saponin, flavonoid, polyphenol, triterpenoid, anthocyanin, ursolic acid and carbonate could be expected to help accelerate the process of wound healing, especially to reduce tissue death. The content of CMC in hydrogel basic can increase TNF-a gene expression that can be increase lymphocytes and neutrophyl to endothelial cells. Necrosis is dead tissue that found in wound because morphological prossces there are three indicators, an increase number of cell debris, PMN (Polymorphonuclear) and decrease in plasma cells. This study was to determine the effect of topical wound care hyperglycemia using hydrogels binahong to decrease necrosis in rat male Strain Wistar. This study true-experiment post-test using post test control group design. The sample were 30 rats take with randomized design and divided into six groups: 4 group binahong hydrogels with concentration 2,5% (n=5), 5% (n=5), 7,5% (n=5), control (-) NS care (n=5) and control (+) using hydrogel base. The measured data are necrosis indicators of a decrease in post-treatment wound for 12 days. Analysis Kruskal Walls p=0,000 (plasma cells) and p=0,006 (PMN) ((p< 0,05). Post Hoc Test p = 0.002 in necrosis (cell debris), p = 0.000 (plasma cells), p = 0.005 (PMN) (p <0.05). It can be concluded that using a combination of hydrogel binahong decrease tissue necrosis n hyperglicemia wound.
