Abstract

Hampir 20 juta anak balita menderita Severe Acute Malnutrition (SAM) dan sebagian besar terjadi di Asia yaitu 0,6 juta di Afghanistan, 0,6 juta di Bangladesh, 8,0 juta di India, 1,2 juta di Indonesia, 1,4 juta di Pakistan, dan 0,6 juta di Yaman Hal ini sesuai dengan laporan tahunan bahwa cakupan mencapai angka kesembuhan minimum yaitu 75% dan proporsi cakupan sekitar 20% dalam menjangkau malnutrisi pada anak. Banyak aspek yang berkontribusi atas implementasinya dalam system kesehatan berbagai Negara dengan kebijakan berbeda. Penelitian ini bertujuan untuk mengetahui implementasi / penerapan Community Based Management of Acute Malnutrition (CMAM) pada system kesehatan, terkait hambatan, tantangan, peluang serta efektivitasnya dengan metode Narrative Literature Review. Berdasarkan hasil temuan jurnal utama yang telah melakukan penelitian di Bangladesh dan Pakistan, didapatkan hasil bahwa dalam implementasi CMAM ada banyak sekali faktor yang berkontribusi. Hal tersebut sesuai dengan 6 Building Block dari WHO tentang system kesehatan yaitu terkait Nutrition Governant, Nutrition Financing, pelayanan, sumber daya manusia, peralatan dan perlengkapan serta mekanisme pelaporan gizi. Dari hal tersebut, didapatkan masing masing hambatan, tantangan, peluang serta efektivitas CMAM dari tiga komponen inti CMAM yaitu Jangkauan Masyarakat, Rawat Jalan dan Rawat Inap.Timbul tema tema baru, yang mempengaruhi 6 inti Building Block dari WHO tersebut dan sesuai dengan lima elemen kunci integrasi CMAM. Kesimpulan dari penelitian ini adalah pada implementasinya, CMAM dirasa sudah cukup efektif dalam menjangkau malnutrisi dinilai dari komponen CMAM yang cukup mumpuni dan berkesinambungan untuk menciptakan optimalnya mengatasi malnutrisi pada anak. Diperlukan pendekatan dari sector pemerintah, sehingga implementasinya oleh system kesehatan kepada masyarakat juga dapat lebih optimal.

English Abstract

Nearly 20 million children under five suffer Severe Acute Malnutrition (SAM) and most of it occurs in Asia, namely 0.6 million in Afghanistan, 0.6 million in Bangladesh, 8.0 million in India, 1.2 million in Indonesia, 1.4 million in Indonesia. 1,4 million in Pakistan, and 0.6 million in Yemen. This is in accordance with the annual report that coverage reaches a minimum cure rate of 75% and the proportion of coverage is around 20% in reaching child malnutrition. Many aspects contribute to its implementation in the health systems of various countries with different policies. This study aims to determine the implementation of Community Based Management of Acute Malnutrition (CMAM) in the health system, related to obstacles, challenges, opportunities and effectiveness with the Narrative Literature Review method. Based on the findings of major journals that have conducted research in Bangladesh and Pakistan, it was found that in the implementation of CMAM there are many contributing factors. This is in accordance with the WHO's 6 Building Blocks regarding the health system, which are related to Nutrition Governant, Nutrition Financing, services, human resources, equipment and supplies as well as nutrition reporting mechanisms. From this, each obstacle, challenge, opportunity and effectiveness of CMAM was obtained from the three core components of CMAM, namely Community Outreach, Outpatient and Inpatient. New themes emerged, which influenced the 6 core Building Blocks of WHO and were in accordance with the five key elements. CMAM integration. The conclusion of this study is that in its implementation, CMAM is considered to be quite effective in reaching malnutrition, judging from the components of CMAM which are quite capable and sustainable to create optimally overcoming malnutrition in children. An approach from the government sector is needed, so that its implementation by the health system to the community can also be more optimal.
