Abstract
Inflammatory Bowel Disease (IBD) merupakanpenyakitperadanganusus yang memilikigejalaantaralain, diare, sembelit, rasa nyeripadaperut, sendawadankembung. Salah satupenyebab IBD adalahpenggunaanNon Steroidal Anti- Inflammatory Drugs (NSAIDs) sepertiindometasin. Indometasinmerupakanobat yang banyakdigunakanuntukpengobatan rheumatoid arthritis. Dalamaksikerja, indometasinakanmenghambat cyclooxygenase 1 (COX-1) yang berperandalampembentukanprostalglandinpadausus. Penurunan prostaglandin menyebabkanpenurunanperlindunganterhadapmukosabarierusus, sehinggamemudahkaninvasibakteripatogen. Pengobatan IBD secarakonvensionaldilakukandenganpemberianobat-obatandarijeniskortison (steroid), anti- inflamasi, penekansystemimundanantibiotik. Air perakmemilikisifatalamisebagaiantiinflamasi, antibakteridanantijamur. Penelitianinibertujuanuntukmengetahuiefek air peraksebagaialternatif terapi IBD padatikusputih (Rattusnorvegicus) hasilinduksiindometasinterhadapgambaranhistopatologi jejunum danekspresitnf-alfa. Penelitianinimenggunakan lima kelompokperlakuanyaitucontrolnegatif, controlpositif (indometasin), kelompokterapidengan air perakdengandosis 25 ppm masing-masingsebanyak 1 mL, 2 mL, dan 3 mL. Parameter penelitianiniadalahgambaranhistopatologi jejunum dianalisissecaradeskriptifkualitatifdanpengukuranekspresi TNF-alfa jejunum dideskripsidengan software Immunoratiokemudiandata dianalisa denganuji ANOVA dilanjutkandenganuji BNJ α= 0,05danmemperolehhasiluji yang menunjukkanbahwatikus IBD yang diterapi air perak(Ag) mengalamipenurunanekspresi TNF-Alfa. Pengamatangambaran histologi jejunum dengan pewarnaan HE dianalisa secara deskripsi kualitatif berdasarkan kerusakan jaringan mukosa jejunum. Hasil penelitian menunjukkan bahwa terapi air perak secara signifikan (p<0,05) mampu menurunkan ekspresi TNF-Alfa dan memperbaiki histopatologi jejunum dengan volume pemberian 3mL karena fase penyembuhan masih berada pada fase proliferasi (Hari ke-11 pasca pemberian terapi air perak Ag).
English Abstract
Inflammatory Bowel Disease (IBD) is an intestinal inflammatory disease that has symptoms including diarrhea, constipation, pain in the stomach, belching and bloating. One of the causes of IBD is the use of Non Steroidal Anti- Inflammatory Drugs (NSAIDs) such as indomethacin. Indomethacin is a widely used drug used for the treatment of rheumatoid arthritis. In action, indomethacin inhibits cyclooxygenase 1 (COX-1) which plays a role in the formation of glandin in the intestine. Decreasing prostaglandin causes a decrease in protection of the intestinal barrier mucosa, thereby facilitating the invasion of pathogenic bacteria. Conventional IBD treatment is done by administering medicines from cortisone (steroid), anti-inflammatory, immune system suppressants and antibiotics. Silver water has natural properties as anti-inflammatory, antibacterial and antifungal. This study aims to determine the effect of silver water as an alternative therapy for IBD in rat (Rattus norvegicus) indomethacin induction on TNF-alpha expression and jejunum histopathology. This study used five treatment groups, namely negative control, positive control (indomethacin), therapy group with silver water with a dose of 25 ppm each as much as 1 mL, 2 mL, and 3 mL. The parameters of this study were jejunum histopathology images analyzed qualitatively and the measurements of TNF-alpha jejunum expression described by Immunoratio software and then ANOVA test followed by BNJ test α = 0.05and obtained test results showing that IBD rats treated with silver water (Ag) had decreased TNF-Alfa expression. Observation of jejunum histology with HE staining was analyzed in a qualitative description based on the damage of jejunum mucosa tissue. The results showed that silver water therapy significantly (p <0.05) was able to reduce TNF-Alfa expression and improve jejunum histopathology with 3mL administration volume and 3mL administration volume could be used as an alternative to IBD therapy in white rats (Rattus norvegicus).
