Abstract

Pneumonia yang muncul dalam 48 jam atau lebih setelah mengalami perawatan di rumah sakit disebut Hospital-acquired pneumonia (HAP). Di Indonesia case fatality rate HAP lebih tinggu daripada community acquired pneumonia. Penggunaan antibiotik golongan cephalosporins generasi ketiga pada pasien HAP yang disebabkan oleh Escherichia coli memicu terjadinya HAP yang disebabkan oleh ESBL producing Escherichia coli. Clinical outcome dari infeksi oleh bakteri multiresisten tersebut meliputi: mortalitas, perpanjangan length of stay, perpanjangan durasi penggunaan cephalosporins generasi ketiga dan perburukan klinis. Belum ada publikasi tentang hubungan antara HAP yang disebabkan oleh ESBL producing Escherichia coli dengan clinical outcome di RSUD Dr Saiful Anwar Malang. Tujuan penelitian ini untuk mengetahui adanya hubungan antara HAP yang disebabkan oleh ESBL producing Escherichia coli dengan clinical outcome pasien di RSUD Dr. Saiful Anwar Malang. Penelitian ini menggunakan rancangan analitik observasional dengan desain case-control study dengan jumlah sampel 15 kasus dan 28 kontrol. Hasil analisis univariat menunjukkan HAP yang disebabkan oleh ESBL producing Escherichia coli tidak berhubungan secara bermakna dengan mortalitas, LOS, durasi penggunaan cephalosporins generasi ketiga dan perburukan (p = 0,309; p = 0,441; p = 0,062; p = 0,392). Case fatality rate HAP yang disebabkan ESBL producing Escherichia coli pada pasien usia lebih dari 60 tahun lebih tinggi dibandingkan dengan non ESBL producing Escherichia coli (p = 0,029). Kesimpulan dari penelitian ini adalah tidak ada hubungan antara kejadian HAP yang disebabkan ESBL producing Escherichia coli dengan clinical outcome, namun tingkat keparahannya lebih tinggi pada pasien usia lanjut.

English Abstract

Pneumonia that appears within 48 hours or more after undergoing treatment in a hospital is called Hospital-acquired pneumonia (HAP). In Indonesia, case fatality rate HAP is more high than community acquired pneumonia. The use of third-generation cephalosporins antibiotics in patients with HAP caused by Escherichia coli triggers the occurrence of HAP caused by ESBL producing Escherichia coli. Clinical outcomes of infections by these multiresistent bacteria include: mortality, extended length of stay, extended duration of use of third generation cephalosporins and clinical deterioration. There is no publication yet about the relationship between HAP caused by ESBL producing Escherichia coli with clinical outcome at RSUD Dr Saiful Anwar Malang. The purpose of this study was to determine the relationship between HAP caused by ESBL producing Escherichia coli with the clinical outcome of patients in Dr. Saiful Anwar Malang. The research uses an observational analytical design with case-control study with 15 cases of samples and 28 controls. The results of the univariate analysis showed that the HAP caused by ESBL producing Escherichia coli was not related meaningfully with mortality, LOS, the duration of third-generation cephalosporins and the clinical decay (p = 0.309; p = 0.441; p = 0.062; p = 0.392). The case fatality rate of HAP caused by ESBL producing Escherichia coli in patients over 60 years of age is higher compared to non-ESBL products. The conclusion of this study is that there is no relationship between the incidence of HAP due to ESBL producing Escherichia coli with clinical outcome, but the severity is higher in older patients.
