Abstract

Ventilator associated pneumonia (VAP) merupakan pneumonia yang terjadi setelah 48 jam pemakaian ventilasi mekanik. Acinetobacter baumanii (A. baumanii) merupakan patogen penyebab VAP terbanyak dengan pilihan terapi antibiotik yang sangat terbatas Penelitian ini bertujuan untuk membuktikan adanya hubungan antara VAP yang disebabkan oleh Carbapenem resistant A. baumanii (CRAB) dengan clinical outcome pasien di unit perawatan intensif RSUD Dr Saiful Anwar Malang berdasarkan data rekamtmedis pasien VAP yang di rawat di unit perawatan intensif RSUD Dr Saiful Anwar Malang dari bulan Juni 2018 hingga Juni 2019. Analisis univariat (uji chi-square) dan analisis multivariat (multiple logistic regression) dilakukan terhadap beberapa clinical outcome yaitu sepsis, mortalitas, morbiditas, durasi pemakaian ventilator di UPI, dan durasi perawatan di UPI. Hasil analisis statistik menunjukkan bahwa pasien VAP 15 kali lebih berpotensi mengalami sepsis dibandingkan pasien VAP disebabkan Carbapenem susceptible A. baumannii (p = 0,008, OR: 15, 95% CI 2,024- 111,174). Disimpulkan bahwa VAP yang disebabkan oleh CRAB berhubungan dengan terjadinya sepsis pada pasien yang dirawat di unit perawatan intensif RSUD Dr Saiful Anwar Malang.

English Abstract

Ventilator associated pneumonia (VAP) is pneumonia that occurs after 48 hours of mechanical ventilation. Acinetobacter baumanii (A. baumanii) is the most pathogenic cause of VAP with a very limited choice of antibiotic therapy. This study aimed to prove the association between VAP caused by Carbapenem resistant A. baumanii (CRAB) and the clinical outcome of patients hospitalized in the intensive care unit in Dr. Saiful Hospital Anwar Malang based on medical record data of VAP patients treated in the intensive care unit of Dr. Saiful Anwar Malang Hospital from June 2018 to June 2019. Univariate analysis (chi-square test) and multivariate analysis (multiple logistic regression) were performed to the following clinical outcomes, namely sepsis, mortality, morbidity, duration of ventilator use, and duration of treatment at ICU. Statistical analysis showed that VAP patients were 15 times more likely to develop sepsis than VAP patients due to Carbapenem susceptible A. baumannii (p = 0.008, OR: 15, 95% CI 2.024- 111.174). It is concluded that VAP were associated with sepsis among patients hosptalized in the intensive care unit of Dr. Saiful Anwar Hospital Malang.
