Abstract
Ventilator Associated Pneumonia (VAP) merupakan pneumonia yang terjadi setelah 48 jam pemakaian ventilasi mekanik. Carbapenem resistant Acinetobacter baumanii (CRAB) merupakan patogen penting penyebab VAP dengan pilihan terapi antibiotik yang sangat terbatas sehingga meningkatkan mortalitas dan biaya perawatan. Faktor risiko VAP yang disebabkan oleh CRAB di Indonesia belum diketahui. Penelitian ini bertujuan untuk menganalisis hubungan antara faktor risiko pasien meliputi jenis kelamin, usia, durasi penggunaan ventilator, durasi perawatan di UPI, riwayat reintubasi, riwayat penyakit penyerta pasien, durasi penggunaan antibiotik empirik meropenem dan skor APACHE II dengan VAP yang disebabkan oleh CRAB di unit perawatan intensif RSUD Dr Saiful Anwar Malang. Rancangan penelitian ini adalah observasional analitik dengan desain case-control. Sampel diambil dengan tehnik consecutive sampling dengan jumlah kasus sebanyak 12 pasien dan jumlah kontrol sebanyak 12 pasien dengan melihat rekamtmedis pasien VAP yang di rawat di unit perawatan intensif RSUD Dr Saiful Anwar Malang dari bulan Juni 2018 hingga Juni 2019. Hasil analisis statistik menunjukkan bahwa terdapat hubungan antara lama perawatan pasien lebih dari 8 hari serta penggunaan meropenem lebih dari 5 hari dengan VAP yang disebabkan oleh CRAB (berturutturut p = 0,024 dan p = 0,032; OR = 10 dan OR = 17,9; 95% CI = 1,360-81,053 dan 95% CI = 1,267-250,000). Disimpulkan bahwa lama perawatan di unit perawatan intensif RSUD Dr Saiful Anwar Malang lebih dari 8 hari dan penggunaan meropenem lebih dari 5 hari merupakan faktor risiko terjadinya VAP yang disebabkan oleh CRAB di RSUD Dr Saiful Anwar Malang.
English Abstract
Ventilator Associated Pneumonia (VAP) is pneumonia that occurs after 48 hours of mechanical ventilation. Carbapenem resistant Acinetobacter baumanii (CRAB) is an important pathogen causing VAP with very limited choice of antibiotic therapy thereby increasing mortality and treatment costs. The risk factors for VAP caused by CRAB in Indonesia are unknown. This study aims to analyze the relationship between patient risk factors including gender, age, duration of ventilator use, length of stay, history of reintubation, history of comorbidities of patients, duration of meropenem use as empirical therapy, and APACHE II score with VAP caused by CRAB in the intensive care unit in Dr. Saiful Anwar hospital in Malang. The design of this study was observational analytic with case-control design. Samples were taken by consecutive sampling technique with a total of 12 patients and a total of 12 patients by seeing the records of VAP patients treated in the intensive care unit of Dr. Saiful Anwar Hospital Malang from June 2018 to June 2019. The statistical analysis showed that there was a relationship between length of stay more than 8 and meropenem use more than 5 days with VAP caused by CRAB (p = 0.024 and p = 0,032; OR = 10 and OR = 17,9; 95% CI = 1,360-81,053 and 95% CI = 1,267- 250,000, respectively). It is concluded that staying in intensive care unit more than 8 days and using meropenem more than 5 days were risk factors for VAP caused by CRAB in Dr Saiful Anwar hospital in Malang.
