Abstract
Asesmen pra-anestesi merupakan bagian penting dari keselamatan pelayanan anestesi namun angka kepatuhannya masih rendah. Salah satu faktor penyebabnya ialah masalah komunikasi dalam tim yang merupakan komponen utama dari kolaborasi interprofesional. Penelitian ini bertujuan untuk mengkaji hubungan antara pengetahuan tentang asesmen pra-anestesi dan kolaborasi serta sikap kolaborasi terhadap perilaku kolaborasi perawat dan dokter dalam mendukung asesmen pra-anestesi pada kasus operasi elektif, serta menggambarkan proses asesmen pra-anestesi. Pengumpulan data dilakukan dengan survei dan observasi yang diperkuat data rekam medik. Survei dilakulan pada 80 dokter dan perawat yang mengukur pengetahuan, sikap, dan perilaku serta praktek pendelegasian kewenangan. Observasi dilakukan selama 11 hari dengan total 45 kasus operasi. Analisis path dilakukan untuk membuktikan hubungan antar variabel, sedangkan praktek pendelegasian dan asesmen pre anestesi dianalisis secara deskriptif. Hasil penelitian menunjukkan pengetahuan tidak mempunyai hubungan yang signifikan, sedangkan sikap mempunyai hubungan negatif yang signifikan terhadap perilaku. Demikian juga pengetahuan mempunyai hubungan negatif lemah terhadap sikap. Secara keseluruhan model hanya bisa menjelaskan 22% variasi perilaku kolaborasi subjek penelitian. Pada sebagian besar (80,5%) kasus operasi elektif, tidak dilakukan asesmen pra-anestesi serta terdapat peralihan tugas pada beberapa aspek pelaksanaan asesmen pra-anestesi di ruangan, terutama pada aspek meminta persetujuan informed consent kepada pasien dan keluarganya (57,5%). Dapat disimpulkan bahwa asesmen pra-anestesi belum terlaksana dan terjadi pendelegasian tugas pada perawat. Pengetahuan bukan menjadi faktor yang beperan pada sikap dan perilaku kolaborasi inter profesi dalam asesmen pra anestesi.
English Abstract
Eventhough pre-anesthesia assessment is crucial to ensure the safety of anesthesia services but its compliance rates are low. One of the contributing factors is communication problems within the team which are a major component of interprofessional collaboration. This study aims to examine the relationship between knowledge of pre-anesthesia assessment and collaboration, and collaborative attitudes towards nurse and physician collaboration behavior in supporting pre-anesthesia assessment of elective surgery. In addition, study also intended to describe the pre-anesthesia assessment practice. A cross sectional design was performed using surveys, observations, and medical record evaluation. The survey was conducted to measure knowledge, attitudes, and behavior, and the practice of task shifting with 80 doctors and nurses as respondents. Furthermore, we performed pre-anesthesia assessment practice observation within 11 days with a total of 45 surgery cases. Path analysis was carried out to identify the relationship between variables, while the practice of delegation and pre-anesthesia assessment was analyzed descriptively The results of the study show that knowledge does not have a significant relationship, whereas attitudes have a significant negative relationship to behavior. Likewise, knowledge has a negative relationship with attitude. Overall the model can only explain 22% of the variation in collaboration behavior of the research subjects. In most cases of elective surgery, no pre-anesthesia assessment (80.5%) was carried out and there was a shift in assignments in several aspects of the implementation of pre-anesthesia assessments in the room, the largest in the aspect of requesting informed consent to patients and their families (57.5%). It can be concluded that implementation of pre-anesthesia assessment was not completely performed supported with the practice of task shifting form doctor to nurset. Knowledge is not a factor that plays a role in inter-professional collaborative attitudes and behaviors in pre-anesthesia assessment.
