Abstract

Inflamasi metabolik (low-grade inflammation) merupakan faktor kunci etiopatogenesis pada perkembangan sindroma metabolik. Nuclear Factor Kappa Beta (NF-κβ) adalah regulator transkripsi gen yang berperan dalam imunitas, respon inflamasi dan dapat dihubungkan dengan keadaan patologis terkait obesitas seperti perlemakan hati nonalkoholik (NAFLD). NF-κβ teraktivasi oleh berbagai stimulus, seperti stres metabolik (hiperglikemia, ROS, metabolisme lemak) dan sitokin proinflamasi (TNF-α, IL-6, IL-1β). Penelitian ini bertujuan untuk menyelidiki mekanisme molekuler yang mendasari NAFLD pada tikus yang diberi diet HFHF (High Fat High Fructose) modifikasi AIN-93M. Studi penelitian eksperimental dengan post test only controlled group design dilakukan pada 30 ekor tikus Sprague Dawley jantan (usia ± 3 bulan, berat badan 200-250 gram) yang dibagi menjadi 2 kelompok perlakuan (P1: diet standar AIN-93M; P2: diet HFHF modifikasi AIN-93M) melalui teknik randomisasi rancangan acak lengkap (RAL). Dilakukan pembedahan setelah 17 minggu masa perlakuan, ekspresi NF-κβ dinilai menggunakan metode immunohistokimia (skor IRS). Hasil penelitian menunjukkan bahwa terdapat perbedaan yang signifikan dalam asupan pakan dan asupan energi antara kelompok P1 dengan P2 (p= 0,000, p= 0.000). Dibandingkan dengan kelompok kontrol (P1), rata-rata ekspresi NF-κβ pada kelompok P2 secara signifikan memiliki nilai yang lebih tinggi (p= 0.000). Hasil uji korelasi antara asupan diet dengan ekspresi NF-κβ menunjukkan bahwa terdapat hubungan yang positif antara asupan energi, karbohidrat dan lemak terhadap ekspresi NF-κβ (p= 0.001, 0.000, 0.046). Disisi lain, terdapat hubungan yang negatif antara asupan protein dengan ekspresi NF-κβ (p= 0.000). Kesimpulan dari penelitian ini adalah diet HFHF modifikasi AIN-93M berpengaruh terhadap peningkatan ekspresi NF-κβ pada jaringan hati tikus Sprague Dawley jantan.

English Abstract

Metabolic inflammation (low-grade inflammation) is an etiopathogenic key factor in the development of metabolic syndrome. Nuclear Factor Kappa Beta (NF- κβ) is a transcriptional regulator of genes involved in immunity, inflammatory response and may be linked to pathology associated with obesity, such as nonalcoholic fatty liver disease (NAFLD). NF-κβ can be activated by various stimuli (metabolic stress (hyperglycmeia, ROS, fat metabolism) and proinflammatory cytokines (TNF-α, IL-6, IL-1β)). The aim of this study was to investigate the molecular mechanisms underlying NAFLD in rats fed high fat high fructose diet (modification of AIN-93M). Experimental studies with post test only controlled group design were carried out on thirty 3-month-old male Sprague Dawley rats with 200-250 g in weight, divided into 2 groups by simple random sampling (P1: standard AIN-93M diet; P2: HFHF diet). All group were sacrificed after 17 weeks of treatment and the expression of NF-κβ in the liver were assessed using immunohistochemistry methods (IRS score). The results showed there were a significant differences in pellet intake and energy intake between P1 group and P2 group (p= 0,000, p= 0.000). Compared with the control group (P1), the average of NF-κβ expression were significantly higher in treatment group (P2) (p= 0.000). The result of correlation test between dietary intake and the expression of NF-κβ showed that there was a positive relationship between the intake of energy, carbohydrates and fats on NF-κβ expression (p= 0.001, 0.000, 0.046), but there was a negative relationship between protein intake and NF-κβ expression in the liver (p= 0.000). In conclusion, high fat high fructose diet (modification of AIN-93M) increases the nuclear factor kappa beta (NF-κβ) expression in the liver tissue of male Sprague Dawley rat.
