Abstract
PT Terminal Teluk Lamong anak perusahaan dari PT Pelabuhan Indonesia III (persero) bergerak di bidang logistik pelayanan jasa bongkar muat peti kemas dan curah kering. Perusahaan ini memiliki beberapa area, salah satunya adalah container yard. Container yard (CY) merupakan lapangan penumpukan peti kemas, berisiko tinggi yang dapat merugikan proses bisnis hingga membahayakan keselamatan manusia. CY operasionalnya sudah otomatis dan bergantung dengan sistem yang bernama Terminal Operation System (TOS) dan tingkat mobilitasnya tinggi. Oleh karena itu tujuan penelitian ini untuk menganalisis risiko operasional di area container yard PT Terminal Teluk Lamong sehingga dapat mengetahui perbaikan yang diperlukan dari permasalahan ini. Metode penelitian yang digunakan pada penelitian ini dengan metode Failure Mode and Effect Analysis dan Root Cause Analysis. Tahap pertama yang dilakukan dalam penelitian ini yaitu mengumpulkan data primer dan data sekunder. Data primer berupa hasil wawancara dan diskusi dengan keyperson serta data nilai severity, occurance, dan detection risiko operasional. Sedangkan data sekunder berupa profil perusahaan, kegiatan operasional, jumlah peti kemas perusahaan tahun 2015-2017, data perkembangan bongkar muat barang tahun 2002-2016, dan data persentase penyebab kegagalan di CY tahun 2017. Kemudian dilakukan pengolahan data yang terbagi menjadi empat tahap. Pertama, identifikasi risiko proses operasional di area CY berdasarkan alur proses. Kedua, analisis risiko dengan Failure Mode and Effect Analysis. Ketiga, analisis akar penyebab risiko kritis dengan Root Cause Analysis dengan 5-why. Keempat, mitigasi risiko kritis. Penelitian ini menunjukkan total risiko yang teridentifikasi ada 22 risiko. Kemudian didapatkan 15 risiko kritis bedasarkan prinsip pareto. Risiko kritis yang memiliki nilai RPN tertinggi adalah (R12) operator ASC tidak segera menghentikan proses operasional saat kecepatan angin>18 m/s. Sedangkan risiko kritis yang memiliki nilai RPN terendah adalah (R9) risiko sistem Terminal Operating System down. Rekomendasi yang diberikan untuk risiko kritis yang memiliki RPN tertinggi adalah membuat warning light, melakukan briefing kerja secara rutin tiap sebelum dan sesudah shift kerja, pelatihan khusus pekerja secara berkala, dibuat safety sign, dan dibuat surat pernyataan kepada customer. Saran yang diberikan yaitu perusahaan diharapkan dapat menerapkan rekomendasi perbaikan yang telah diusulkan, rutin melakukan pencatatan setiap permasalahan, serta untuk penelitian selanjutnya diharapkan dapat menambah metode lain agar lebih baik lagi dan dilakukan penelitian analisis risiko perusahaan untuk ruang lingkup yang lebih luas.
English Abstract
PT Terminal Teluk Lamong, a subsidiary of PT Pelabuhan Indonesia III (Persero), is engaged in logistics of loading and unloading container services and dry bulk services. This company has several areas, one of which is container yard. Container yard (CY), a container stacking field, is a high risk area that can harm business processes to endanger human safety. CY is operational automatically and relies on a system called Terminal Operation System (TOS) and its mobility is high. Therefore, the aim of this research was to analyze operational risk in the container area of PT Terminal Teluk Lamong to help create the necessary improvements of this problem. The research method used in this research is Failure Mode and Effect Analysis and Root Cause Analysis. The first stage of this research was collecting primary data and secondary data. Primary data is the result of interviews and discussions with keyperson and data on the value of severity, occurance, and detection of operational risks. While secondary data in the form of company profile, operational activities, number of company containers in 2015-2017, data on increasing the number of loading and unloading goods in 2002-2016, and data on the percentage of causes of failure in CY in 2017. Data processing is then divided into four stages. First, identification of operational process risks in the CY area based on the process flow. Second, risk analysis used Failure Mode and Effect Analysis. Third, root cause analysis of critical risk used Root Cause Analysis method with 5-why. Forth, critical risk mitigation. This research shows that the total risks identified were 22 risks. Then there were 15 critical risks based on the Pareto principle. The critical risk that has the highest RPN value is (R12) ASC operators do not immediately stop the operational process when the wind speed is more than 18 m /s. While the critical risk that has the lowest RPN value is (R9) the risk of Terminal Operating System down. The recommendation given for critical risk that has the highest RPN is to make a warning light, conduct work briefings regularly before and after work shifts, periodic training for workers, make safety signs, and make a statement letter to the customer. The advice given is that the company is expected to be able to implement recommendations for improvements that have been proposed, routinely record every problem, and for further research is expected to add other methods to be better and conducted risk analysis research for a wider scope.
