Abstract

Pasien skizofrenia merupakan penyakit gangguan jiwa yang menyebabkan beban pada keluarga dalam pemenuhan perawatan dan pengobatannya. Beban yang meningkat memicu keluarga untuk berespon dengan marah serta mengurung pasien. Respon tersebut menunjukkan mekanisme koping keluarga maladaptif. Beban dan mekanisme koping maladaptif berdampak pada kualitas hidup keluarga dan kemampuan keluarga dalam merawat pasien skizofrenia. Tujuan penelitian ini adalah untuk membuktikan pengaruh terapi suportif kelompok terhadap beban keluarga dan mekanisme koping keluarga dalam merawat anggota keluarga skizofrenia. Desain penelitian ini adalah Quasy experimental pre post test with control group design. Jumlah sampel sebanyak 14 pada kelompok perlakuan dan 15 pada kelompok kontrol dengan tehnik purposive sampling. Instrumen beban keluarga menggunakan The Zarit Burden Interview (ZBI) serta instrumen mekanisme koping dengan Brief COPE Inventory (BCI). Terapi suportif ini dilakukan selama 6 kali pertemuan dalam 4 sesi dengan jeda waktu 1 hari pada tiap sesinya. Analisis data menggunakan uji T-Test. Hasil penelitian menunjukkan terdapat perbedaan nilai beban dan mekanisme koping antara sebelum dan sesudah terapi suportif kelompok pada kelompok perlakuan (nilai p-value = 0,000). Pada kelompok kontrol terdapat perbedaan nilai beban antara sebelum dan sesudah penyuluhan kesehatan (nilai p-value = 0,010), sedangkan tidak terdapat perbedaan nilai mekanisme koping antara sebelum dan sesudah penyuluhan kesehatan (p-value = 0,225). Hasil akhir penelitian didapatkan perbedaan yang signifikan pada nilai beban antara kelompok perlakuan dan kontrol sesudah terapi suportif kelompok (p-value = 0,002) dan terdapat perbedaan nilai mekanisme koping pada perlakuan dan kontrol sesudah terapi suportif kelompok (p-value = 0,001). Terapi suportif lebih mempengaruhi mekanisme koping dengan nilai R-square sebesar 0,328. Kesimpulan penelitian ini menunjukkan adanya pengaruh terapi suportif kelompok terhadap beban keluarga dan mekansime koping keluarga dalam merawat anggota keluarga dengan skizofrenia. RSUD dr Rehatta Jepara diharapkan memfasilitasi adanya pembentukan peer support group pada poli jiwa sehingga derajat kesehatan jiwa keluarga dapat tercapai.

English Abstract

Schizophrenic patients are psychiatric illness that causes the burden on the family in the fulfillment of their care and treatment. The increased burden triggers the family to respond angrily and confine the patient. The response shows the coping mechanism of the maladaptive family. The burden loads and maladaptive coping mechanisms have an impact on the quality of family life and the ability of families to care for schizophrenic patients. The purpose of this study was to prove the effect of supportive group therapy on family burden and coping mechanisms in caring for family members who suffer schizophrenia. The design of this research is Quasy experimental pre post test with control group design. The number of samples were 14 in the treatment group and 15 in the control group with purposive sampling technique. The family load instrument uses The Zarit Burden Interview (ZBI) as well as a coping mechanism instrument using COPE Inventory Brief (BCI). This supportive therapy was conducted during 6 sessions in 4 sessions with 1 day interval at each session. Data analysis using T-Test. The results showed that there were differences on load values and coping mechanisms before and after supportive group therapy in the treatment group (p-value = 0,000). In the control group, there were differences on the value of load before and after health counseling (p-value = 0,010). Whereas, there was no difference of coping mechanism value before and after health counseling (p-value = 0,225). The result of the study showed significant differences on the load values between treatment and control groups after supportive group therapy (p-value = 0.002) and the differences on the value of coping mechanisms on treatment and control after supportive group therapy (p-value = 0.001). Supportive therapy more influence coping mechanism with R-square value of 0.328. The conclusions of this study indicate the effect of supportive group therapy on family burden and family coping mechanisms in treating family members with schizophrenia. Dr Rehatta Hospital is expected to facilitate the formation of peer support group in the psychiatry clinic so mental health in the family can be achieved.
