Abstract

Fibrosis hati adalah reaksi penyembuhan luka pada hati, yang ditandai dengan akumulasi jaringan matriks ekstraseluler. Dalam perkembangannya, fibrosis hati dapat menyebabkan berbagai komplikasi, antara lain; sirosis hati, gagal hati, dan kanker. Tahap akhir fibrosis sendiri, yakni sirosis hati, menjadi penyebab utama morbiditas dan mortalitas penyakit hati di dunia. Fibrosis hati akibat induksi karbon tetraklorida (CCl4) dapat menyebabkan kerusakan pada hati, ginjal, paru, dan susunan saraf pusat. Luka yang terjadi secara terus menerus akan mengaktivasi berbagai sitokin proinflamasi salah satunya yaitu Interleukin-17 (IL-17), sebagai hasil sekresi dari subset baru famili sel T CD4+, yaitu Th 17, yang telah dibuktikan dalam berbagai penelitian memiliki kontribusi yang besar pada kejadian fibrosis hati. Kurkumin, senyawa polifenol, telah digunakan sebagai anti-oksidan dan anti-inflamasi. Selain itu kurkumin telah diindikasikan sebagai obat yang potensial untuk fibrosis hati. Senyawa ini menurunkan ekspresi mediator pro-inflamasi, mengurangi stress oksidatif, serta menghambat aktivasi dan mengatur apoptosis dari Hepatic Stellate Cells (HSC). Penelitian ini bertujuan untuk mencari tahu hubungan pemberian kurkumin dengan proses penyembuhan fibrosis hati. Hewan coba dibagi menjadi delapan kelompok perlakuan dengan waktu paparan CCl4 selama 9 minggu kemudian dilanjutkan dengan pemberian kurkumin secara oral pada kelompok perlakuan dan pemberian pelarut kurkumin pada kelompok kontrol dengan lama pemberian yang sama selama 2, 5, dan 9 minggu. Diakhir perlakuan, IL-17 serum diukur menggunakan metode Enzyme-linked Immunosorbent Assay (ELISA) dan derajat fibrosis hati dinilai dengan mikroskop pembesaran 200x setelah dicat menggunakan metode Hematoxilin Eosin (HE). Didapatkan hasil bahwa hubungan kadar IL-17 serum dengan derajat fibrosis hati memiliki korelasi yang lemah (r = 0.353).

English Abstract

Liver fibrosis is an injury healing response in the liver, which is marked by the accumulation of extracellular matrix tissue. In its development, liver fibrosis can cause a variety of complications, among others; liver cirrhosis, liver failure and cancer. The final stage of fibrosis is liver cirrhosis, is a major cause of morbidity and mortality of liver disease in the world. Fibrosis induced by carbon tetrachloride (CCl4) can cause damage to the liver, kidneys, lungs, and central nervous system. Injuries that occur continuously activates various proinflammatory cytokines one of them is Interleukin-17 (IL-17), as a result of the secretion of a subset of new families of CD4+ T cells: Th 17, which has been demonstrated in numerous studies to have a great contribution in the progression of liver fibrosis. Curcumin, a polyphenol compounds, has been used as an anti-oxidant and anti-inflammatory. Besides, curcumin has been indicated as a potential treatment for liver fibrosis. This substance may decrease the expression of pro-inflammatory mediators, reduce oxidative stress, as well as inhibit the activation and regulates apoptosis of Hepatic stellate Cells (HSC). This study aims to find out the relationship giving curcumin with the healing process of liver fibrosis. Animals were divided into eight groups treated with CCl4 exposure time for 9 weeks and then followed by oral administration of curcumin in the treatment group and solvent administration of curcumin on a control group with a duration of the same for 2, 5, and 9 weeks. At the end of treatment, serum IL-17 were measured using enzyme-linked immunosorbent assay (ELISA) and the degree of liver fibrosis assessed with 200x magnification microscope after painted using Hematoxilin Eosin (HE) method. This study shows that animals treated with curcumin have less IL-17 compared to control group. The result of this study is the relationship of serum levels of IL-17 with a degree of liver fibrosis has weak correlation (r = 0353).
