Abstract

Malnutrisi masih menjadi masalah yang tidak terselesaikan di rumah sakit. Beberapa penelitian telah membuktikan bahwa malnutrisi yang terjadi di rumah sakit mengakibatkan menurunnya fungsi otot, fungsi respirasi, fungsi kekebalan tubuh (imunitas), dan gangguan penyembuhan luka. Melihat seriusnya dampak malnutrisi yang terjadi di rumah sakit maka perlu dilakukan skrining gizi pada pasien baru yang akan menjalani rawat inap di rumah sakit. Tujuan dari penelitian ini adalah untuk mengetahui perbandingan sensitivitas dan spesifisitas skrining gizi antara SGA ( Subjective Global Assessment ) dan MST ( Malnutrition Screening Tool ) dengan parameter kadar albumin di RS HVA Toeloengredjo Pare. Penelitian deskriptif analitik dengan desain Cross-Sectional digunakan untuk menilai risiko malnutrisi dari 70 pasien di ruang rawat inap RS HVA, dengan umur 44,5 (21-59) tahun. Berdasarkan SGA, 44,3% pasien berstatus gizi baik, 50% pasien berstatus gizi kurang dan 5,7% berstatus gizi buruk. Berdasarkan MST, 41,4% pasien tidak berisiko malnutrisi dan 58,6% berisiko malnutrisi. Subjective Global Assessment memiliki sensitivitas yang cukup baik 71,1%, spesifisitas 72%, nilai prediksi positif (PPV) 82,1% dan nilai prediksi negatif (NPV) 58,1%. Sedangkan MST memiliki sensitivitas yang lebih baik yaitu 86,7%, spesifisitas 92%, nilai prediksi positif (PPV) 95,1% dan nilai prediksi negatif (NPV) 79,3%. Uji korelasi spearman menunjukkan MST memiliki korelasi yang lebih kuat dengan albumin (p = 0,000; r = 0,757) dibandingkan dengan SGA (p = 0,000; r = 0,473). Malnutrition Screening Tool memiliki sensitivitas dan spesifisitas yang lebih tinggi daripada SGA sehingga dapat digunakan sebagai rekomendasi alat skrining yang dapat secara cepat mengidentifikasi malnutrisi pada pasien yang baru masuk rumah sakit.

English Abstract

Malnutrition remains a problem that is not resolved in the hospital. Several studies have shown that malnutrition in hospitalized patients is generally related to decreased muscle function, respiratory function, immune function, and impaired wound healing. The impact of malnutrition in hospital was a serious problem, so nutrition screening should be conducted in new patients who will undergo inpatient hospital. The aim of this study was to compare the sensitivity and specificity of nutritional screening tool between SGA (Subjective Global Assessment) and MST (Malnutrition Screening Tool) with albumin serum level as parameter at HVA Hospital Toeloengredjo Pare. Analytic descriptive study with cross-sectional design was used to assess the risk of malnutrition of 70 hospitalised patient in the HVA hospital, with age 44,5 (21-59) years. According to SGA, 44,3% of patients were well nourished, 50% of patients were moderately malnourished and 5,7% severely malnourished. According to MST, 41,4% patients not at risk of malnutrition and 58,6% patients at risk of malnutrition. Subjective global assessment had fair sensitivity 71,1%, specificity 72%, positive predictive value (PPV) 82,1%, and negative predictive value (NPV) 58,1%. While MST had a better sensitivity 86,7%, specificity 92%, positive predictive value (PPV) 95,1%, and negative predictive value (NPV) 79,3%. Spearman correlation test showed that MST has a stronger correlation with albumin serum level (p = 0,000; r = 0,757) compared with SGA (p = 0,000; r = 0,473). Malnutrition screening tool has higher sensitivity and specificity than SGA that can be used as a screening tool recommendation that can quickly identify malnutrition in patients newly admitted to hospital.
