Abstract

Latar Belakang: Maloklusi merupakan kasus yang banyak ditemui pada praktek dokter gigi di Indonesia. Salah satu cara untuk memperbaiki maloklusi adalah dengan memakai peranti ortodonsia lepasan. Keberhasilan peranti lepasan tergantung pada kemauan pasien untuk memakai peranti dan kemampuan operator untuk mendesain dan membuat peranti yang dapat ditoleransi pasien. Keparahan maloklusi dapat diukur dengan menggunakan indeks. Index of Orthodontic Treatment Needs (IOTN) adalah indeks yang digunakan untuk menyatakan keadaan kebutuhan perawatan ortodonti. IOTN terdiri dari dua komponen yaitu Dental Health Component (DHC) dan Aesthethic Index (AC). Sudah pernah dilakukan beberapa penelitian dengan DHC untuk mengukur keparahan maloklusi, tetapi apakah DHC dapat digunakan sebagai indeks pengukur efektivitas perawatan peranti lepasan belum teruji. Tujuan: Penelitian ini bertujuan untuk mengetahui efektivitas DHC untuk mengevaluasi progres perawatan peranti lepasan. Metode: Penelitian ini merupakan penelitian deskriptif analitik observasional dengan pendekatan cross sectional. Sampel penelitian berjumlah 83 model studi dan rekam medik dari tahun 2009-2011 di Universitas Jember. Pengambilan sampel data dilakukan dengan mengukur tingkat keparahan maloklusi model studi sebelum perawatan dan model studi setelah pasien mencapai progres II perawatan dengan menggunakan indeks DHC. Hasil penelitian dianalisa menggunakan uji Wilcoxon Signed Rank untuk membandingkan nilai DHC sebelum dilakukan perawatan dan setelah progres II perawatan. Hasil: Penelitian ini menunjukkan perbedaan nilai DHC secara signifikan antara sebelum dilakukan perawatan dengan setelah progres II perawatan (nilai koefisien Wilcoxon 0.000, p >0.05) ketika dianalisis dengan menggunakan uji Wilcoxon Signed Rank. Kesimpulan: Hasil pengukuran menggunakan DHC menunjukkan adanya perbaikan maloklusi setelah progres II perawatan. DHC efektif digunakan sebagai indeks evaluasi progres perawatan peranti lepasan.

English Abstract

Background: Malocclusion is frequently found in dental practice in Indonesia. One way to correct malocclusion is to use orthodontic removable appliances. The success of the treatment depends on the patients’ cooperation to wear the removable appliances and the operators ability to design and make a device that can be tolerated by the patients. The severity of malocclusion can be measured using an index. Index of Orthodontic Treatment Needs (IOTN) that is an index used to declare a state of orthodontic treatment need. IOTN consists of two components namely Dental Health Component (DHC) and Aesthethic Index (AC). Many researches had been done to measure the severity of the malocclusion, however whether DHC could be used as an index to measure the effectivity of orthodontic treatment using removable appliances has not been tested. Purpose: This aims of study is to determine the effectiveness of DHC index to evaluate removable appliances progress of treatment. Methods: This study was a descriptive cross sectional observational. The sample used were 83 study models and medical records from 2009-2011 at the University of Jember. Data sampling was done by measuring the severity of the malocclusion from the study models taken before treatment compared to the study models taken after second progress of treatment using DHC index. The results of the study was analyzed using the Wilcoxon Signed Rank test to compare the value of DHC before treatment and after the second progress of treatment. Results: This study showed a significant difference between the measurement taken before treatment compared with the second progress of treatment (Wilcoxon coeficient 0.000, p > 0.05) when analyzed using the Wilcoxon Signed Rank test. Conclusion: The measurements using DHC showed better results after second progress of treatment. DHC effectively used as an evaluation index of removable appliances progress of treatment.
