Abstract

Penanganan masalah kesehatan jiwa telah bergeser dari Hospital based menjadi Community based psychiatric services, sehingga pelayanan tidak hanya berfokus terhadap upaya kuratif tetapi lebih menekankan upaya proaktif yang berorientasi pada upaya pencegahan (preventif) dan promotif (WHO, 2013). Salah satu upaya yang dilakukan dalam menangani masalah kesehatan jiwa masyarakat adalah dengan pengembangan Desa Siaga Sehat Jiwa (DSSJ) yang merupakan pengembangan program kesehatan mental berbasis masyarakat (Keliat et al., 2011). Gangguan jiwa di Indonesia menunjukkan bahwa prevalensi gangguan jiwa berat di daerah pedesaan ternyata lebih tinggi dibandingkan daerah perkotaan (Riskesdas, 2015), sehingga program DSSJ menjadi salah satu solusi tepat dalam mengatasi masalah tersebut. Terwujudnya DSSJ tentunya membutuhkan peran serta kader (Marchira, 2014). Berdasarkan hal tersebut maka tujuan dalam penelitian ini adalah untuk mengeksplorasi pengalaman kader jiwa dalam mewujudkan DSSJ. Penelitian ini merupakan penelitian kualitatif dengan pendekatan fenomenologi intepretif. Penelitian ini dilakukan di 5 desa berbeda yang termasuk dalam wilayah kerja Puskesmas Kecamatan Bantur. Partisipan dalam penelitian ini adalah kader jiwa yang aktif dan telah ditunjuk sebagai kader DSSJ dari awal dibentuk DSSJ (tahun 2011-2012) hingga saat ini (2017) didapatkan sebanyak 6 partisipan. Data dikumpulkan melalui wawancara mendalam dengan panduan wawancara semi testruktur kemudian dianalisis dengan menggunakan Intepretive Phenomenological Analysis (IPA) (Smith et al, 2009). Dari hasil penelitian ini diperoleh 7 tema. Tujuh tema tersebut meliputi: membesarkan hati dan niat menjadi kader demi menjawab kepercayaan masyarakat; prihatin belum optimalnya dukungan semua pihak; rasa syukur atas keberhasilan memenuhi tanggung jawab sebagai kader; iba dengan pasien yang mendapatkan stigma negatif dari masyarakat; gigih menjalankan tugas sebagai kader; meyakini memiliki tanggung jawab moral membantu sesama sebagai ladang pahala; serta mendambakan kepedulian semua pihak. Sehingga dari tujuh tema besar tersebut, core tema dalam penelitian ini adalah Kegigihan mewujudkan niat sukarela membantu sesama melalui tanggung jawab sebagai kader. Kader DSSJ memiliki motivasi diri yang besar dalam menjalankan perannya. Hal tersebut telah terbukti dengan masih terus aktif membantu pasien dan keluarga hingga saat ini. Kesimpulan dalam penelitian ini ialah niat tulus dan rasa tanggung jawab tinggi yang dimiliki kader menjadi kunci kegigihan kader dalam melaksanakan tugas membantu pasien dan keluarga, sehingga dengan kegigihan tersebut memberikan manfaat terwujudnya program DSSJ di wilayah kerja Puskesmas Kecamatan Bantur.

English Abstract

The handling of mental health problems has shifted from hospital-based to communitybased psychiatric services, so that service is not just for curative efforts more pressing proactive efforts that are oriented to prevention and promotive efforts (WHO, 2013). One of the efforts undertaken in public health problems is the development of Desa Siaga Sehat Jiwa (DSSJ) which is the development of community based mental health programs (Keliat et al., 2011). Mental disorders in Indonesia show the prevalence of mental illness in rural areas is higher than urban areas (Riskesdas, 2015), so the DSSJ program becomes one of the right solutions to solve the problem. The realization of DSSJ certainly requires of Lay mental health volunteer’s roles (Marchira, 2014). Based on that, the purpose of this research is to experience the soul cadre in realizing DSSJ. This research is a qualitative research with intepretif phenomenology approach. This research was conducted in 5 different villages included in the work area of Bantur District Health Center. Participants in this research are active cadre of soul and have been appointed as DSSJ cadre from the beginning of DSSJ (2011-2012) until now (2017) get 6 participants. Robust data through interviews with semi-structured interview guides were then analyzed using the Intenretological Phenomenological Analysis (IPA) (Smith et al, 2009). The results of this study are 7 themes. The themes include encouragement and intention to become a lay mental health volunteer to answer the trust of the community; concern about the support from all parties which have not been optimum yet; gratitude for successfully fulfilling the responsibilities as a lay mental health volunteer; compassionate to the patients who get negative stigma from community; persistently performing duties as a lay mental health volunteer; believes to have a moral responsibility to help others as fields of reward; as well as yearning the concern of all parties. Thus, the core theme in this research, obtained from the seven big themes, is the persistence to create voluntary intention to help others through the responsibility as a lay mental health volunteer. The great self-motivation of the DSSJ lay mental health volunteer in performing their role has been proven by still actively helping patients and families today. The tasks and roles that have been performed even though the number of missed ones has yielded positive results. In conclusion, sincere intentions and high sense of responsibility of the lay mental health volunteers are the keys to the lay volunteer's perseverance in carrying out the duty to help the patient and family so that the DSSJ in the work area of Bantur District Public Health Center could be realized.
