Abstract

Fibrosis terjadi sebagai respon perbaikan jaringan rusak akibat adanya injury. Bleomisin merupakan salah satu obat anti-kanker yang dapat menginduksi fibrosis paru pada manusia dan hewan. Terbentuknya fibrosis tidak lepas dari pengaruh imunitas tubuh terutama sitokin Th2 dalam meregulasi remodeling jaringan dan fibrosis. Perubahan fibrosis dapat diamati secara histopatologi menggunakan metode Masson’s Trichrome. Kefir susu kambing merupakan bahan pangan fungsional yang mengandung bakteri asam laktat, khamir (yeast) dan metabolitnya seperti peptida aktif, eksopolisakarida serta β-galaktosidase. Tujuan penelitian ini adalah untuk membuktikan kefir sebagai imunomodulator terhadap penghambatan keparahan fibrosis paru subkronis pada tikus yang diinduksi bleomisin. Penelitian eksperimental ini menggunakan 20 ekor tikus wistar jantan, umur 10-12 minggu dengan berat badan 129,5 ± 17,34 g. Semua kelompok tikus dipajan dengan bleomisin intranasal sebanyak 9 kali selama 9 hari. Percobaan terdiri atas 4 kelompok perlakuan: Kelompok (K-) tanpa pajanan kefir, (P1) kelompok pajanan kefir 2,5 ml, (P2) kelompok pajanan kefir, 3,5 ml dan (P3) kelompok pajanan kefir 4,5 ml. Kefir diberikan dengan dosis per berat badan tikus per hari selama 30 hari. Pengamatan histopatologi dengan pewarnaan Masson’s trichrome, untuk mengamati rata-rata ketebalan deposisi matrik ekstraseluler (fibrosis) pada peribronkial. Hasil analisis menunjukkan bahwa terdapat penurunan ekspresi ketebalan fibrosis peribronkial pada kelompok P1, P2, dan P3. Ketebalan P1 tidak jauh berbeda dengan kelompok kontrol, sementara itu pada P3 mengalami penebalan fibrosis yang paling tipis namun secara statistik tidak signifikan. Kesimpulan bahwa kefir kemungkinan memiliki aktivitas sebagai inhibitor fibrogenesis untuk mengurangi keparahan fibrosis paru akibat aplikasi bleomisin pada tikus.

English Abstract

Fibrosis is an accumulation of excessive extracellular matrix proteins in response to tissue repair damaged by injury. Bleomycin is one of the anti-cancer drugs that can induce lung fibrosis in humans and animals. The formation of fibrosis can’t be separated from the influence of body immunity, especially Th2 cytokines in regulating tissue remodeling and fibrosis. The change of fibrosis can be observed histopathologically using Masson's Trichrome method. Goat milk kefir is a functional food that contains lactic acid bacteria, yeast and its metabolites such as active peptides, exopolysaccharides and β-galactosidase. The objective of this study was to prove kefir as an immunomodulator against inhibition of subcronic pulmonary fibrosis severity in bleomycin-induced rats. This experimental study used 20 male wistar rats, aged 10-12 weeks weighing 129.5 ± 17.34 g. All of the rats were exposed to intranasal for 9 times during 9 days. The experiments consist of 4 treatment groups: Group (K-) without exposure to kefir, (P1) exposure group 2.5 ml kefir, (P2) 3.5 ml kefir, and (P3) 4.5 ml kefir/g BW/day for 30 day. Observation of histopathology by staining of Masson's trichrome, to observe the average thickness of extracellular matrix deposition (fibrosis) in the peribronchial. The results showed that there was decreased expression of peribronchial fibrosis thickness in P1, P2, and P3 groups. The thickness of P1 was not significantly different with the control group, while P3 experienced the slightest but statistically insignificant thickening of fibrosis. Conclusion that kefir may have activity as a fibrogenesis inhibitor to reduce the severity of pulmonary fibrosis due to bleomycin application in rats.
