Efek Terapi Kombinasi Doxorubicin dan Ekstrak Daun Zodia (Evodia suaveolens) terhadap Ekspresi Microtubule-associated Protein 4 (MAP4) dan Siklus Sel Fase G2/M Acute Myeloid Leukemia HL-60 in vitro

Abstract

Leukemia myeloid akut (AML) merupakan keganasan hematologi yang mematikan dengan pilihan terapi yang terbatas. Penelitian ini menyelidiki efek sinergis doxorubicin dan ekstrak daun Evodia suaveolens terhadap ekspresi protein terkait mikrotubulus 4 (MAP4) dan penghentian siklus sel pada sel AML HL-60. Ekstrak daun E. suaveolens dibuat dengan maserasi menggunakan etanol 96%. Sel HL-60 diberikan perlakuan dengan doxorubicin 0,2 μg/ml saja atau dalam kombinasi dengan berbagai konsentrasi ekstrak daun E. suaveolens (0.2, 0.4, 0.8 mg/ml). Ekspresi MAP4 dinilai dengan imunofluoresensi, dan siklus sel fase G2/M dianalisis secara kuantitatif dengan flow cytometry dengan pewarnaan propidium iodida. Pemberian kombinasi doxorubicin dengan ekstrak daun E. suaveolens dosis tinggi (0,8 mg/ml) secara signifikan mengurangi ekspresi MAP4 dibandingkan dengan kontrol negatif dan doxorubicin saja, dengan efek yang bergantung pada dosis. Analisis siklus sel menunjukkan bahwa pemberian perlakuan kombinasi doxorubicin dengan ekstrak daun E. suaveolens memengaruhi distribusi siklus sel HL-60 fase G2/M. Doxorubicin saja tidak dapat menurunkan akumulasi fase G2/M secara signifikan disbanding kontrol, sementara kombinasi doxorubicin dan ekstrak E. suaveolens dosis tinggi (0.4 dan 0.8 mg/ml) dapat menghambat akumulasi sel HL-60 fase G2/M secara signifikan. Kombinasi doxorubicin 0.2 μg/ml dan ekstrak daun E. suaveolens 0.8 mg/ml secara sinergis meningkatkan efek antikanker pada sel HL-60 melalui penurunan ekspresi MAP4 dan siklus sel fase G2/M. Studi ini memberikan dasar untuk mengembangkan terapi kombinasi yang lebih efektif dan mengidentifikasi senyawa bioaktif baru dalam ekstrak E. suaveolens untuk pengembangan obat antikanker, terutama AML, yang potensial.

English Abstract

Acute myeloid leukemia (AML) is a deadly hematological malignancy with limited therapeutic options. This study investigated the synergistic effects of doxorubicin and Evodia suaveolens leaf extract on microtubule-associated protein 4 (MAP4) expression and cell cycle arrest in AML HL-60 cells. E. suaveolens leaf extract was prepared by maceration using 96% ethanol. HL-60 cells were treated with 0.2 μg/ml doxorubicin alone or in combination with various concentrations of E. suaveolens leaf extract (0.2, 0.4, 0.8 mg/ml). MAP4 expression was assessed by immunofluorescence, and G2/M phase cell cycle was quantitatively analyzed by flow cytometry with propidium iodide staining. The combination of doxorubicin with high dose E. suaveolens leaf extract (0.8 mg/ml) significantly reduced MAP4 expression compared to negative control and doxorubicin alone, with a dose-dependent effect. Cell cycle analysis showed that the combination treatment of doxorubicin with E. suaveolens leaf extract affected the distribution of HL-60 cell cycle in the G2/M phase. Doxorubicin alone could not significantly reduce the accumulation of the G2/M phase compared to the control, while the combination of doxorubicin and high doses of E. suaveolens extract (0.4 and 0.8 mg/ml) could significantly inhibit the accumulation of HL-60 cells in the G2/M phase. The combination of doxorubicin 0.2 μg/ml and E. suaveolens leaf extract 0.8 mg/ml synergistically increased the anticancer effect on HL-60 cells by decreasing MAP4 expression and the G2/M phase cell cycle. This study provides a basis for developing more effective combination therapies and identifying new bioactive compounds in E. suaveolens extract for the development of potential anticancer drugs, especially AML.
