Abstract

Latar Belakang: Pekerja industri marmer berisiko terpajan debu yang mengandung kristal silika di tempat kerja. Debu silika berkontribusi pada sebagian besar kasus penyakit paru okupasional. Neopterin yang merupakan biomarker awal dari respon imun seluler diketahui memiliki kaitan dengan pajanan debu silika. Penelitian ini ditujukan untuk menganalisis perubahan kadar neopterin berdasarkan lama pajanan debu silika. Metode: Penelitian analitik potong lintang dengan subjek pekerja industri marmer ini mengikutsertakan 9 subjek dengan pajanan 1-5 tahun, 9 subjek dengan pajanan 5-10 tahun, 9 subjek dengan pajanan >10 tahun, dan 9 subjek tidak terpajan (kontrol). Kadar neopterin yang dianalisa dengan metode ELISA dilakukan uji beda dan korelasi berdasarkan lama pajanan. Hasil: Subjek memiliki rerata usia 39,19 + 10,20 tahun. Ditemukan perbedaan kadar neopterin yang signifikan (p<0,05) antar kelompok lama pajanan. Ditemukan perbedaan kadar neopterin yang signifikan antara kontrol dengan keseluruhan pekerja (p<0,05). Kadar neopterin juga ditemukan signifikan lebih tinggi pada subjek dengan lama pajanan 5-10 tahun dibanding kontrol (p<0,05). Kadar neopterin juga ditemukan signifikan lebih tinggi pada subjek dengan lama pajanan >10 tahun dibanding kontrol (p<0,05). Uji korelasi juga menunjukkan adanya korelasi positif dan signifikan antara kadar neopterin dengan lama pajanan (r=0,615; p<0,05). Kesimpulan: Lama pajanan debu silika berpengaruh signifikan terhadap kadar neopterin serum, terutama pada lama pajanan lebih dari 5 tahun. Semakin lama pekerja industri marmer terpajan debu silika, semakin tinggi kadar neopterin serumnya.

English Abstract

Background: Marble industry workers are at risk of exposure to dust containing silica crystals in the workplace. Silica dust contributes to the majority of cases of occupational lung diseases. Neopterin, which is an early biomarker of cellular immune response, is known to be associated with silica dust exposure. This study aimed to analyze the changes in neopterin levels based on the duration of silica dust exposure. Method: This analytical cross-sectional study included 9 subjects with 1-5 years of exposure, 9 subjects with 5-10 years of exposure, 9 subjects with >10 years of exposure, and 9 unexposed subjects (controls) from the marble industry workers. Neopterin levels were analyzed using the ELISA method, and differences and correlations based on the duration of exposure were analyzed. Results: The subjects had a mean age of 39.19 + 10.20 years. Significant differences in neopterin levels (p<0.05) were found between the different duration of exposure groups. Significant differences in neopterin levels (p<0.05) were found between control group and all workers (p<0,05). Neopterin levels were also significantly higher in subjects with 5-10 years of exposure compared to controls (p<0.05). Neopterin levels were also significantly higher in subjects with >10 years of exposure compared to controls (p<0.05). The correlation test also showed a significant positive correlation between neopterin levels and duration of exposure (r=0.615; p<0.05). Conclusion: The duration of silica dust exposure significantly affects serum neopterin levels, especially for exposures longer than 5 years. The longer marble industry workers are exposed to silica dust, the higher their serum neopterin levels.
