Abstract

Latar Belakang: Karsinoma nasofaring (KNF) merupakan salah satu jenis kanker kepala dan leher yang prevalensinya tinggi di Asia, termasuk Indonesia. Proses onkogenesis pada KNF tipe undifferentiated melibatkan berbagai macam faktor molekuler, termasuk kerusakan DNA akibat stress oksidatif dan mekanisme antiapoptosis. 8-hydroxy-2'-deoxyguanosine (8-OHdG) dipakai sebagai penanda kerusakan oksidatif DNA dan B-cell lymphoma 2 (BCL-2) dipakai sebagai penanda mekanisme anti-apoptosis dalam suatu proses onkogenesis. Tujuan: Mengetahui korelasi antara ekspresi 8-OHdG dan ekspresi BCL-2 terhadap proses onkogenesis pada KNF tipe undifferentiated. Metode: Penelitian observasional analitik dengan pendekatan cross-sectional yang melibatkan 27 sampel biopsi jaringan nasofaring dari penderita KNF tipe undifferentiated. Pemeriksaan ekspresi 8-OHdG dan BCL-2 menggunakan metode imunohistokimia yang dinilai oleh ahli Patologi Anatomi. Hasil: Ekspresi 8-OHdG dengan staging tumor primer menunjukkan korelasi yang bermakna (p=0,029), akan tetapi tidak bermakna dengan staging nodul (p=0,357) dan stadium klinis (p=0,36). Ekspresi BCL-2 tidak menunjukan korelasi yang bermakna dengan staging tumor primer (p=0,11), staging nodul (p=0,181) dan stadium klinis (p=0,424). Kesimpulan: Korelasi ekspresi 8-OHdG lebih bermakna daripada kolerasi ekspresi BCL-2 pada proses onkogenesis KNF tipe undifferentiated terutama pada proses proliferasi sel.

English Abstract

Background: Nasopharyngeal carcinoma (NPC) is a head and neck cancer with a high prevalence in Asia, including Indonesia. The oncogenesis process in undifferentiated NPC involves a wide variety of molecular factors, including DNA damage due to oxidative stress and anti-apoptosis mechanisms. 8-hydroxy-2'- deoxyguanosine (8-OHdG) is used as a marker of oxidative damage to DNA and B-cell lymphoma 2 (BCL-2) is used as a marker of anti-apoptosis mechanisms in an oncogenesis process. Objective: To determine the correlation between 8- OHdG and BCL-2 expression on oncogenesis process in undifferentiated NPC. Methods: An observational analytical study with a cross-sectional approach involving 27 nasopharyngeal tissue biopsy samples from patients with undifferentiated type NPC. Examination of 8-OHdG and BCL-2 expression using immunohistochemical methods assessed by Anatomical Pathologists. Results: The expression of 8-OHdG with primary tumor staging showed a significant correlation (p=0.029), but not significant with nodule staging (p=0.357) and clinical stage (p=0.36). BCL-2 expression did not show a significant correlation with primary tumor staging (p=0.11), nodule staging (p=0.181) and clinical stage (p=0.424). Conclusion: The correlation of 8-OHdG expression is more significant than the correlation of BCL-2 expression in the oncogenesis process of undifferentiated type NPC, especially in cell proliferation process.
