Abstract
Rekonsiliasi obat yang tidak lengkap dapat menyebabkan risiko kesalahan dalam manajemen penggunaan obat dan merugikan pasien. Penelitian ini bertujuan untuk mengetahui hubungan faktor individu, organisasi, dan psikologis terhadap kelengkapan pengisian formulir rekonsiliasi obat di RSUD Dr. H. Slamet Martodirdjo Pamekasan menggunakan pendekatan analisis multilevel. Penelitian ini menggunakan metode penelitian dengan desain cross-sectional. Analisa penelitian ini menggunakan teknik multilevel logistik regresi, sampel dikelompokkan menjadi dua level yaitu, level apoteker dan level formulir rekonsiliasi obat pada pasien. Sampel pada level apoteker adalah seluruh apoteker sebanyak 5 apoteker sedangkan populasi pada level formulir rekonsiliasi obat sebanyak 5.472 formulir rekonsiliasi obat dengan teknik purposive sampling. Sampel diperoleh sebanyak 372 formulir rekonsiliasi obat. Alat ukur instrumen menggunakan kuesioner. Hasil penelitian menunjukkan Apoteker dengan pengetahuan yang sangat baik lebih banyak mengisi formulir rekonsiliasi obat. Semakin tinggi faktor organisasi dan faktor psikologis secara simultan meningkatkan kelengkapan pengisian formulir rekonsiliasi obat. Kesimpulan apoteker dengan pengetahuan yang tinggi memiliki kontribusi positif terhadap kelengkapan pengisian formulir rekonsiliasi obat. Pentingnya faktor psikologis dan faktor organisasi dalam meningkatkan pengisian formulir juga menjadi poin kunci, menekankan peran motivasi dan keterlibatan personal apoteker serta sistem reward and punishment dalam menjalankan tugas rekonsiliasi obat.
English Abstract
Incomplete medication reconciliation can lead to the risk of errors in medication use management and harm patients. This study aims to determine the relationship between individual, organizational and psychological factors on the completeness of filling out the medication reconciliation form at RSUD Dr. H. Slamet Martodirdjo Pamekasan uses a multilevel analysis approach. This research uses research methods with a cross-sectional design. This research analysis used a multilevel logistic regression technique, the sample was grouped into two levels, namely, pharmacist level and patient medication reconciliation form level. The sample at the pharmacist level was all 5 pharmacists, while the population at the drug reconciliation form level was 5,472 drug reconciliation forms using a purposive sampling technique. Samples obtained were 372 medication reconciliation forms. The instrument measuring tool uses a questionnaire. The results showed that pharmacists with excellent knowledge filled out more medication reconciliation forms. The higher levels of organizational factors and psychological factors simultaneously increase the completeness of filling out the medication reconciliation form. Conclusion: Pharmacists with high knowledge have a positive contribution to the completeness of filling out the medication reconciliation form. The importance of psychological factors and organizational factors in improving form filling is also a key point, emphasizing the role of motivation and personal involvement of pharmacists as well as reward and punishment systems in carrying out medication reconciliation tasks.
