Abstract

Tujuan: Mengetahui peningkatan kualitas hidup pasien thyroid eye disease (TED) setelah pemberian intravena methylprednisolone pulse terapi. Metode: Penelitian dengan desain analitik observasional kohort dengan pendekatan prospektif. Dilakukan di Poli Mata RSUD Dr. Saiful Anwar – Malang dari bulan Januari-Desember 2023. Semua pasien TED yang berobat jalan yang mendapat intravena methylprednisolone pulse (IVMP) terapi.dilakukan wawancara menggunakan kuesioner GO-QOL yang sudah diterjemahkan ke dalam Bahasa Indonesia. Wawancara dilakukan sebelum pemberian IVMP dan setelah pemberian IVMP. Data kemudian dilakukan uji statitik menggunakan uji Wilcoxon, one way ANOVA, mann whitney dan t dependent. Hasil: Dalam penelitian ini melibatkan 21 responden yang terdiri dari 15 perempuan dan 6 laki-laki dengan usia rata-rata 40 tahun. Nilai GO-QOL (median) setelah mendapat IVMP pulse terapi untuk fungsi visual dan penampilan masing-masing adalah 81,3 dan 76,7. Nilai GO-QOL berbeda signifikan setelah mendapat IVMP pulse terapi untuk fungsi visual dan penampian (P = 0,000). Setelah mendapat IVMP pulse terapi pasien dengan TED derajat sedang berat memiliki nilai penampilan (median) yang secara signifikan lebih rendah (66,7 vs. 87,1, P = 0,002) dibandingkan dengan derajat ringan, tetapi tidak ada perbedaan dalam fungsi visual. Derajat keparahan pasien TED menurun secara signifikan setelah mendapat IVMP pulse terapi (P = 0,022). Dalam analisis regresi multivariat dari nilai GO-QOL, tidak didapat hubungan yang signifikan dengan usia, jenis kelamin, pendidikan dan pekerjaan dengan fungsi visual dan penampilan. Kesimpulan: Didapatkan peningkatan kualitas hidup pada pasien thyroid eye disease (TED) setelah pemberian intravena methylprednisolone pulse terapi.

English Abstract

Objective: To determine improvement the quality of life of thyroid eye disease (TED) patients after intravenous methylprednisolone (IVMP) pulse therapy. Method: The study used a prospective observational cohort design. It was conducted at the Ophthalmology Clinic of Dr. Saiful Anwar Hospital – Malang from January to December 2023. All TED patients receiving intravenous methylprednisolone (IVMP) pulse therapy were included. Interviews were conducted using the GOQOL questionnaire, which had been translated into Indonesian. Interviews were carried out before and after the IVMP pulse therapy. The data were then analyzed using statistical tests, including the Wilcoxon test, one-way ANOVA, Mann-Whitney test, and dependent t-test. Results: The study involved 21 respondents, consisting of 15 females and 6 males, with an average age of 40 years. The GO-QOL scores (median) after receiving IVMP pulse therapy for visual function and appearance were 81.3 and 76.7, respectively. There was a significant difference in GO-QOL scores after IVMP pulse therapy for both visual function and appearance (P = 0.000). After receiving IVMP pulse therapy, patients with moderate-to-severe TED had significantly lower appearance scores (median) (66.7 vs. 87.1, P = 0.002) compared to those with mild disease, but there was no difference in visual function. The severity of TED significantly decreased after IVMP pulse therapy (P = 0.022). In the multivariate regression analysis of GOQOL scores, no significant association was found with age, gender, education, and occupation in relation to visual function and appearance. Conclusion: There was an improvement in the quality of life in patients with thyroid eye disease (TED) after the administration of intravenous methylprednisolone pulse therapy.
