Abstract
PT. Piranti Utomo Makmur merupakan perusahaan industri di bidang jasa konstruksi yang memproduksi beton ready mix di Dusun Krajan, Kabupaten Malang. Berdasarkan hasil observasi dan wawancara didapatkan bahwa pelaksanaan program Keselamatan dan Kesehatan Kerja (K3) belum sepenuhnya dilakukan oleh perusahaan maupun pekerja produksi beton ready mix. Selama Oktober 2022 hingga Oktober 2023, terdapat 8 kecelakaan kerja dan 6 gangguan kesehatan yang dialami pekerja. Penelitian ini menggunakan metode Hazard Identification and Risk Assessment (HIRA) dan Root Cause Analysis (RCA). Metode HIRA digunakan untuk mengidentifikasi secara sistematis potensi bahaya, sedangkan metode RCA digunakan untuk menganalisis secara sistematis akar penyebab kecelakaan yang terjadi. Hasil identifikasi bahaya dengan metode HIRA ditemukan sebanyak 24 potensi bahaya dan 34 risiko pada proses produksi beton ready mix yang berasal dari tahap persiapan produksi, penakaran material dan pengadukan beton. Dari 34 risiko yang ada, terdapat 1 risiko rating extreme, 12 risiko rating high, 14 risiko rating moderate dan 8 risiko rating low. Risiko dengan rating high dan extreme dievaluasi dengan metode RCA berdasarkan sumber bahayanya, seperti lingkungan kerja, peralatan, pekerja, dan prosedur kerja. Rekomendasi perbaikan diberikan berdasarkan hierarki pengendalian risiko berupa jadwal pembersihan rutin pada tangga batching plant, pemasangan karet pada pijakan tangga, pemberian handrail pada kedua sisi tangga, pemasangan pengamanan mesin pada mesin gearbox, rambu keselamatan dan kesehatan kerja, safety talk, dan rekomendasi APD yang sesuai dengan kebutuhan pekerja.
English Abstract
PT Piranti Utomo Makmur is an industrial company in the field of construction services that produces ready mix concrete in Krajan Hamlet, Malang Regency. Based on the results of observations and interviews, it is found that the implementation of the Occupational Safety and Health (K3) program has not been fully carried out by the company and ready mix concrete production workers. During October 2022 to October 2023, there were 8 work accidents and 6 health problems experienced by workers. This research uses the Hazard Identification and Risk Assessment (HIRA) and Root Cause Analysis (RCA) methods. The HIRA method is used to systematically identify potential hazards, while the RCA method is used to systematically analyze the root causes of accidents that occur. The results of hazard identification with the HIRA method found a total of 24 potential hazards and 34 risks in the ready mix concrete production process originating from the production preparation stage, material weighing and concrete mixing. From 34 existing risks, there are 1 extreme rating risk, 12 high rating risks, 14 moderate rating risks and 8 low rating risks. Risks with high and extreme ratings were evaluated using the RCA method based on their sources of danger, such as the work environment, equipment, workers, and work procedures. Improvement recommendations are given based on the risk control hierarchy in the form of a routine cleaning schedule on the batching plant stairs, installation of rubber on the steps of the stairs, provision of handrails on both sides of the stairs, installation of machine guards on the gearbox engine, occupational safety and health signs, safety talks, and PPE recommendations that suit the needs of workers.
