Abstract
Tn. D mengalami luka tekan pada bagian punggung yang disebabkan oleh immobilitas berkepanjangan. Sebelum masuk rumah sakit, Tn. D jatuh di kamar mandi dan menyebabkannya mengalami imobilitas selama 6 bulan. Pada saat pengkajian di rumah sakit kondisi kulit Tn. D kering, tidak elastis, mengalami edema, dan turgor kulit kembali dalam waktu 2 detik. Bundle of Decubitus yang terdiri dari 4 kegiatan utama yaitu analisis kondisi kulit yang dilakukan setiap hari, serta pemberian olive oil, bony prominence protectors, dan positioning untuk mengurangi risiko luka tekan pada pasien bedrest. Tujuan: Penelitian ini bertujuan untuk mengetahui pengaruh pemberian Bundle of Decubitus pada pasien dengan risiko tinggi luka tekan di ruang ICU RSUD Bangil. Metode: Penelitian ini dilakukan dengan menggunakan metode case study yaitu dengan mengambil satu kasus pasien yaitu Tn. D yang diberi intervensi Bundle of Decubitus. Instrumen yang digunakan adalah Braden Scale dan SLKI L.14125 Integritas Kulit dan Jaringan. Setelah didapatkan hasil dilakukan analisis deskriptif untuk ditarik Kesimpulan. Hasil: Sebelum diberi intervensi kondisi kulit pasien kering, tidak elastis, mengalami edema, turgor kulit kembali dalam waktu 2 detik, kemerahan, dan terdapat luka decubitus dengan pus dan bau di punggung. Setelah dilakukan intervensi kondisi kulit lembab, kenyal, licin, terhidrasi dengan baik, elastisitas kulit membaik dimana kulit kembali dalam waktu 1 detik, pitting edema pada ekstremitas berkurang menjadi 2 mm, kemerahan pada luka tekan berkurang, pus dan bau luka tidak ada. Kesimpulan: Pemberian Bundle of Decubitus dapat memperbaiki elastisitas, hidrasi, perfusi jarungan, kerusakan jarungan, kemerahan, suhu, dan tekstur kulit.
English Abstract
Mr. D suffered pressure sores on his back caused by prolonged immobility. Before entering the hospital, Mr. D fell in the bathroom and caused him to experience immobility for 6 months. During the assessment at the hospital, Mr. D is dry, inelastic, edematous, and skin turgor returns within 2 seconds. Bundle of Decubitus which consists of 4 main activities, namely skin condition analysis which is carried out every day, as well as administering olive oil, bony prominence protectors, and positioning to reduce the risk of pressure sores in bedrest patients. Objective: This study aims to determine the effect of administering the Bundle of Decubitus to patients with a high risk of pressure ulcers in the ICU at Bangil Regional Hospital. Method: This research was carried out using the case study method, namely by taking one patient case, namely Mr. D who was given Bundle of Decubitus intervention. The instruments used were the Braden Scale and SLKI L.14125 Skin and Tissue Integrity. After obtaining the results, descriptive analysis was carried out to draw conclusions. Results: Before the intervention was given, the patient's skin condition was dry, inelastic, had edema, skin turgor returned within 2 seconds, redness, and there was a decubitus wound with pus and odor on the back. After the intervention, the skin condition was moist, supple, smooth, well hydrated, skin elasticity improved where the skin returned within 1 second, pitting edema on the extremities was reduced to 2 mm, redness in the pressure wound was reduced, pus and wound odor were absent. Conclusion: Bundle of Decubitus can improve elasticity, hydration, tissue perfusion, tissue damage, redness, temperature and skin texture.
