Abstract
Ruang Pediatric Intensive Care Unit (PICU) merupakan ruang perawatan khusus anak dengan kondisi kritis, penurunan kesadaran dan masalah pernapasan seperti respiratory distress syndrome yang membutuhkan monitoring status hemodinamik. Pneumonia menjadi penyebab terjadinya respiratory distress syndrome dengan gejala sesak nafas, batuk berdahak dan demam yang dapat menyebabkan ketidakstabilan status hemodinamik. Prone position menjadi intervensi paling banyak pada respiratory distress syndrome sehingga dipilih sebagai salah satu implementasi yang bertujuan untuk membahas pengaruh implementasi prone position sebagai salah satu implementasi asuhan keperawatan pada anak dengan respiratory distress syndrome. Implementasi dilakukan dengan metode case study dengan 1 kasus dan 1 intervensi berbasis evidence-based pada 1 pasien di ruang PICU. Hasil penelitian menyatakan bahwa terdapat pengaruh prone position terhadap status hemodinamik seperti saturasi oksigen, frekuensi nadi, frekuensi nafas dan nilai MAP. Saturasi oksigen meningkat secara signifikan dari sebelum implementasi prone position dengan rata-rata SpO2 adalah 88% menjadi 97% setelah prone position. Frekuensi nadi menurun dari 182x/menit sebelum implementasi prone position menjadi 171x/menit setelah prone position. Frekuensi nafas meningkat dari 37x/menit sebelum prone position menjadi 41x/menit setelah prone position. Nilai MAP meningkat dari 61 mmHg sebelum prone position menjadi 67 mmHg setelah prone position. Diharapkan prone position dapat diterapkan pada pasien dengan respiratory distress syndrome atau pada ruangan intensif dengan penggunaan ventilasi mekanik dan kondisi klinis tertentu.
English Abstract
The Pediatric Intensive Care Unit (PICU) is a special care room for children with critical conditions, decreased consciousness and respiratory problems such as respiratory distress syndrome which requires monitoring of hemodynamic status. Pneumonia is the cause of respiratory distress syndrome with symptoms of shortness of breath, cough with phlegm and fever which can cause instability in hemodynamic status. Prone position is the most common intervention for respiratory distress syndrome so it was chosen as one of the implementations which aims to analyze the implementation of prone position as one of the implementations of nursing care for children with respiratory distress syndrome. Implementation was carried out using the case study method with 1 case and 1 evidencebased intervention in 1 patient in the PICU room. The research results stated that there was an influence of prone position on hemodynamic status such as oxygen saturation, pulse frequency, respiratory frequency and MAP value. Oxygen saturation increased significantly from before implementing the prone position with an average SpO2 of 88% to 97% after the prone position. The pulse frequency decreased from 182x/minute before implementing the prone position to 171x/minute after the prone position. Respiratory frequency increased from 37x/minute before prone position to 41x/minute after prone position. The MAP value increased from 61 mmHg before prone position to 67 mmHg after prone position. It is hoped that the prone position can be applied to patients with respiratory distress syndrome or in intensive care units with the use of mechanical ventilation and certain clinical conditions.
