Abstract
Latar Belakang: Epilepsi adalah gangguan neurologis yang mempengaruhi 65 juta orang di seluruh dunia, yang mempengaruhi berbagai aspek kognitif, psikologis, dan sosial. Epilepsi dapat menyebabkan gangguan tidur, dan hubungan antara keduanya bersifat multifaktorial, termasuk salah satunya efek obat anti kejang. Tujuan: Mengetahui perbandingan pengaruh Antiseizure Medication (Fenitoin, Carbamazepine, Asam Valproat) terhadap kualitas tidur berdasarkan Pittsburg Sleep Quality Index pada pasien epilepsi. Metode: Penelitian ini merupakan penelitian yang bersifat observasional dengan metode pengambilan data secara cross sectional dengan sumber data primer yang diperoleh dari semua pasien epilepsi yang mengkonsumsi Antiseizure Medication yang melakukan kunjungan di poliklinik Neurologi RSUD DR. Saiful Anwar Malang. Hasil: Asam Valproat secara signifikan berpengaruh terhadap kualitas tidur dibandingkan dengan fenitoin (OR3.34, p 0.005), Carbamazepin tidak berbeda signifikan dengan fenitoin dalam pengaruh terhadap kualitas tidur (OR 1.16, p 0.78). Carbamazepin tidak berbeda signifikan dengan asam valproat dalam pengaruh terhadap kualitas tidur berdasarkan Pittsburg Sleep Quality Index (OR 2.89, p 0.06). Rerata dosis fenitoin yang tidak menyebabkan gangguan tidur 217 mg dan dosis yang menyebabkan gangguan tidur 260 mg (p 0.005) sehingga dosis pemberian fenitoin juga terkait dengan kualitas tidur, berbeda dengan asam valproate dan Carbamazepine (p 0.308 dan p 0.760) yang tidak memilihi hubungan antara dosis dan kejadian gangguan tidur. Kesimpulan: Asam Valproat memiliki pengaruh paling besar terhadap penurunan kualitas tidur dibandingkan dengan Fenitoin dan Carbamazepine pada pasien epilepsy.
English Abstract
Background: Epilepsy is a neurological disorder that affects 65 million people worldwide, affecting various cognitive, psychological, and social aspects. Epilepsy can cause sleep disturbances, and the relationship between the two is multifactorial, including one of the effects of anti-seizure drugs. Objective: To compare the effect of Antiseizure Medication (Phenytoin, Carbamazepine, Valproic Acid) on sleep quality based on Pittsburg Sleep Quality Index in epilepsy patients. Methods: This study is an observational study with cross sectional data collection methods with primary data sources obtained from all epilepsy patients who take Antiseizure Medication who visit the Neurology polyclinic at RSUD DR. Saiful Anwar Malang. Results: Valproic acid significantly affected sleep quality compared to phenytoin (OR 3.34, p 0.005), Carbamazepine was not significantly different from phenytoin in its effect on sleep quality (OR 1.16, p 0.78). Carbamazepine was not significantly different from valproic acid in the effect on sleep quality based on the Pittsburg Sleep Quality Index (OR 2.89, p 0.06). The mean dose of phenytoin that did not cause sleep disturbance was 217 mg and the dose that caused sleep disturbance was 260 mg (p 0.005) so that the dose of phenytoin administration was also associated with sleep quality, in contrast to valproate acid and Carbamazepine (p 0.308 and p 0.760) which did not choose the relationship between dose and the incidence of sleep disturbance. Conclusions: Valproic acid has the greatest influence on sleep quality compared to Phenytoin and Carbamazepine in epilepsy patients.
