Abstract
Latar Belakang: Tuberkulosis (TB) merupakan penyakit kronis dengan dampak penurunan kualitas hidup dan finansial yang signifikan. Asia Tenggara merupakan penyumbang terbanyak kasus TB. TB resisten obat (TB RO) merupakan jenis TB yang resisten terhadap berbagai pengobatan standar TB. TB RO mengalami tren peningkatan kasus dalam beberapa tahun terakhir. Linezolid yang merupakan salah satu obat lini pertama TB RO diketahui memiliki efek samping myelosupresif yang signifikan, termasuk anemia. Studi ini ditujukan untuk menganalisis perubahan kadar hemoglobin (Hb) dan indeks produksi retikulosit (IPR) pada pasien TB RO yang telah mendapat pengobatan linezolid selama dua bulan. Metode: Penelitian observasional kohort prospektif dengan subjek penderita TB RO yang mendapat terapi linezolid selama dua bulan. Data demografis seperti jenis kelamin, usia, dan status merokok diambil dari rekam medis. Kadar Hb dan IPR dianalisa dari sampel darah tepi sebelum dan setelah dua bulan terapi linezolid. Dilakukan analisa statistik berupa uji korelasi dan uji beda berpasangan. Hasil: Ditemukan penurunan kadar Hb yang signifikan dari baseline hingga dua bulan pasca terapi linezolid (13,27+1,59 vs 11,62 + 1,76, p<0,05). Kemudian, ditemukan penurunan IPR yang signifikan dari baseline hingga dua bulan pasca terapi linezolid (1,59 + 0,5 vs 1,24 + 0,52, p<0,05). Subjek laki-laki ditemukan memiliki kadar Hb baseline yang lebih tinggi (13,95 + 1,32 vs 12,27 + 1,44, p<0,05). Subjek perokok juga ditemukan memiliki kadar Hb baseline yang lebih tinggi (13,93 + 1,29 vs 12,68 + 1,63, p<0,05). Kesimpulan: Penurunan kadar Hb pasca dua bulan terapi linezolid menunjukkan efek linezolid terhadap penurunan eritrosit dalam tubuh. Penurunan IPR pasca dua bulan terapi linezolid menunjukkan adanya penurunan produksi eritrosit, mengarahkan pada efek myelosupresif linezolid.
English Abstract
Tuberculosis (TB) is a chronic disease with significant impact on quality of life and financial aspect. Southeast Asia contributes to the highest number of TB cases. Drug-resistant tuberculosis (DR-TB) is a type of TB that is resistant to various standard TB treatments. Cases of DR-TB have been increasing in recent years. Linezolid, which is one of the first-line drugs for DR-TB, is known to have significant myelosuppressive side effects, including anemia. This study aims to analyze the changes in hemoglobin (Hb) levels and reticulocyte production index (RPI) in DR-TB patients who have received linezolid treatment for two months. Method: This prospective observational cohort study was conducted with subjects diagnosed with DR-TB who received linezolid therapy for two months. Demographic data such as gender, age, and smoking status were obtained from medical records. Hb levels and RPI were analyzed from peripheral blood samples before and after the two-month linezolid therapy. Statistical analysis including correlation tests and paired t-tests were performed. Results: A significant decrease in Hb levels was found from baseline to two months post-linezolid therapy (13.27 + 1.59 vs 11.62 + 1.76, p<0.05). Additionally, a significant decrease in RPI was observed from baseline to two months post-linezolid therapy (1.59 + 0.5 vs 1.24 + 0.52, p<0.05). Male subjects were found to have higher baseline Hb levels (13.95 + 1.32 vs 12.27 + 1.44, p<0.05). Smoking subjects were also found to have higher baseline Hb levels (13.93 + 1.29 vs 12.68 + 1.63, p<0.05). Conclusion: The decrease in Hb levels after two months of linezolid therapy indicates the effect of linezolid in reducing red blood cells in the body. The decrease in RPI after two months of linezolid therapy suggests a decrease in red blood cell production, indicating the myelosuppressive effect of linezolid.
