Abstract

Latar belakang: Efusi pleura maligna menandakan stadium lanjut dari penyakit keganasan dan berhubungan dengan prognosis pasien yang buruk. Biopsi pleura yang bersifat invasif dan rendahnya sensitivitas sitologi cairan pleura menjadi keterbatasan dalam penegakan diagnosis efusi pleura maligna. Pengembangan alat diagnostik non-invasif dan objektif untuk efusi pleura maligna merupakan perspektif yang menjanjikan. MCP- 1 dan LDH merupakan biomarker yang diusulkan sebagai pendekatan untuk mendiagnosis efusi pleura maligna dan diharapkan memberikan ketepatan diagnosis yang lebih baik. Metode: Penelitian dengan desain potong lintang yang melibatkan 88 pasien dengan efusi pleura curiga maligna. Sampel dibagi menjadi dua kelompok, maligna dan non maligna berdasarkan hasil patologi anatomi pleura (sitologi dan/atau biopsi). Sampel cairan pleura dikumpulkan untuk pemeriksaan MCP-1 dan LDH. Hasil: Terdapat perbedaan bermakna antara rata-rata kadar MCP-1 dan LDH pada efusi pleura maligna dan non maligna (p<0.05). Nilai ambang optimal yang ditetapkan untuk MCP-1 adalah 5805.93 pg/mL dan LDH adalah 681.5 U/L. MCP-1 memiliki nilai diagnostik lebih baik dibandingkan LDH. MCP-1 memiliki sensitivitas tertinggi untuk memprediksi kelompok maligna dan non maligna, yaitu sebesar 81.8% (CI 95%: 80.5%-83.1%), dibandingkan LDH sebesar 75.8% (CI 95%: 74.3%-77.3%). Selain itu, MCP-1 memiliki spesifisitas yang paling tinggi sebesar 70.9% (CI 95%: 69.7%-72.1%), dibandingkan LDH sebesar 67.3% (CI 95%: 66.1-68.5%). Disamping itu, terdapat korelasi positif bermakna antara kadar MCP-1 (r=0.490; p=0.001) dan LDH (r=0.242; p=0.023) dengan efusi pleura maligna. Kesimpulan: MCP-1 dan LDH dapat membedakan efusi pleura maligna dan non maligna. Selain itu, didapatkan pula korelasi positif antara MCP-1 dan LDH dengan kejadian efusi pleura malig

English Abstract

Background: Malignant pleural effusion indicates advanced stage of malignancy and associated with poor prognosis. The invasive nature of pleural biopsy and low sensitivity of pleural fluid cytology are challenges in diagnosing malignant pleural effusion. Development of non-invasive and objective diagnostic tools for malignant pleural effusion is promising prospect. MCP-1 and LDH are biomarkers that proposed as approach for diagnosing MPE and are expected to prompt better diagnosis. Methods: Cross-sectional study recruited 88 patients which suspected malignant pleural effusion. Samples were divided into two groups, malignant and nonmalignant based on the result of pathological anatomy result (pleural cytology and/or biopsy). Pleural fluid samples were collected for MCP-1 and LDH examination. Results: There was a significant difference between mean levels of MCP-1 and LDH in malignant and non-malignant pleural effusions (p<0.05). The optimal cut-off value determined for MCP-1 was 5805.93 pg/mL and LDH was 681.5 U/L. MCP-1 had the highest sensitivity for predicting malignant and non-malignant groups, which was 81.8% (CI 95%: 80.5%-83.1%), compared to LDH which was 75.8% (CI 95%: 74.3%- 77.3%). Beside that, MCP-1 had the highest specificity of 70.9% (CI 95%: 69.7%-72.1%, compared to LDH which was 67.3% (CI 95%: 66.1-68.5%). In addition, there was significantly positive correlation between MCP-1 (r=0.490; p=0.001) and LDH (r=0.242; p=0.023) in malignant pleural effusion. Conclusion: MCP-1 and LDH can differentiate malignant and non-malignant pleural effusion. Positive correlation was also found between MCP-1 and LDH in malignant pleural effusion.
