Abstract

Latar Belakang Pasien dengan insufisiensi vena kronis menjalani puncture standar dengan teknik below the knee untuk mengakses vena safena magna (VSM) untuk ablasi laser endovena. Namun, dalam beberapa kasus, sulit untuk melakukan puncture below the knee; puncture above the knee untuk mengakses vena safena yang tidak kompeten adalah pilihan lain. Penelitian ini membandingkan luaran klinis pasien insufisiensi vena kronis setelah EVLA dengan puncture above the knee dan below the knee. Metode/Rancangan Penelitian studi kohort single-centre retrospektif ini membandingkan luaran klinis pasca dua teknik puncture EVLA (puncture above the knee dan below the knee). Pada semua kunjungan follow-up, luaran klinis, tingkat penutupan, dan komplikasi akan dinilai. Hasil Penelitian ini melibatkan 248 pasien, dengan 132 pasien pada kelompok puncture below the knee dan 116 pasien pada kelompok puncture above the knee. Hasil klinis serupa pasca prosedur terlihat pada kedua kelompok dengan peningkatan Venous Clinical Severity Score (VCSS) dan Venous Disability Score (VDS) (p <0,05). Tingkat penutupan pada kedua kelompok adalah 97% (p = 0,595). Komplikasi, termasuk trombosis, memar dan paresthesia tidak menunjukkan signifikansi statistik antar kelompok (p = 1.000, p = 0.101 dan p = 0.667 masing-masing). Cedera luka bakar tidak ada pada kedua kelompok. Kesimpulan EVLA untuk inkompetensi VSM menggunakan teknik puncture above the knee aman dan efektif dan harus dipertimbangkan sebagai metode alternatif jika puncture below the knee gagal pada pasien CVI Non Ulkus.

English Abstract

Background Patients with chronic venous insufficiency (CVI) undergo a standard puncture below the knee to access the great saphenous vein (GSV) for endovenous laser ablation (EVLA). However, in some cases, getting a puncture below the knee is difficult; a puncture above the knee to access the incompetent saphenous vein is another option. This study compared CVI patients' clinical results after EVLA with below- and above-the-knee punctures. Methods/Design This retrospective, single-centre cohort study compared clinical outcomes after two EVLA technical punctures (puncturing the GSV below- and above-knee). At all follow-up visits, clinical outcomes, closure rate, and complications were assessed. Results This study has recruited 248 patients, with 132 patients in the below-knee group and 116 patients in the above-knee group. Similar clinical outcomes after the procedure were seen in both groups with improving of the Venous Clinical Severity Score (VCSS) and the Venous Disability Score (VDS) (p < 0.05). The closure rate in both groups was 97% (p = 0.595). Complications, including thrombosis, bruises and paresthesia did not show statistical significance between groups (p = 1.000, p = 0.101 and p = 0.667 respectively). Burn injury was absent in both groups. Conclusion The EVLA for incompetence GSV using an above-knee puncture was safe and effective and should be considered as an alternative method if the below-knee puncture fails in Non Ulcer CVI patients.
