Abstract
Penelitian ini bertujuan untuk mengetahui hubungan derajat obesitas dengan sifungsi vascular berdasarkan HOMA-IR pada penderita obese. Metode Penelitian kasus kontrol ini dilakukan pada 160 mata yang terbagi 80 kasus dan 80 kontrol. Flux index dan perfusi capiler yang menilai disfungsi pembuluh darah diperiksa dengan OCTA. HOMA-IR digunakan untuk memeriksa resistensi insulin yang menggukana kadar gula darah dan insulin dalam keadaan puasa. Hasil Korelasi negatif ditemukan antara derajat obesitas dengan flux index superior (r=0.173, p=0.029), nasal (r=0.238, p=0.002), inferior (r=0.205, p=0.009) dan outer (r=0.231, p=0.003). Korelasi ini juga ditemukan oada perfusi kapiler inferior (r=0.192, p=0.015), sementara HOMA-IR memiliki korelasi positif (r=0.358, p=0.001). Korelasi negatif rendah yang signifikan secara statistic ditemukan pada perfusi kapiler superior (r=0.225, p=0.004) dan inferior (r=0.222, p=0.005). Kesimpulan Pada obesitas terdapat peningkatan HOMA-IR disertai dengan penurunan flux index dan perfusi kapiler. Menurunnya perfusi kapiler dan flux index menandakan adanya disfungsi vaskular. Sehingga HOMA-IR dapat dipertimbangkan untuk menjadi biomarker disfungsi vascular pada pasien obesitas.
English Abstract
This study aimed to determine the relationship between the degree of obesity and vascular dysfunction based on HOMA-IR. Methods This case-control study was conducted on 160 eyes, divided into 80 cases and 80 controls. Flux index and capillary perfusion, which assess vascular dysfunction were examined by OCTA. HOMA-IR was used to examine insulin resistance, which used glucose levels and insulin under fasting conditions. Result A negative correlation was found between the degree of obesity with superior (r=0.173, p=0.029), nasal (r=0.238, p=0.002), inferior (r=0.205, p=0.009), and outer flux index (r=0.231, p=0.003). This correlation was also found in inferior capillary perfusion (r=0.192, p=0.015) while, HOMA-IR had a positive correlation (r=0.358, p=0.001). The statistically significant low negative correlation was found in HOMA-IR with superior (r=0.225, p=0.004) and inferior (r=0.222, p=0.005) capillary perfusion. Conclusion There was an increase in HOMA-IR accompanied by a decrease in flux index and capillary perfusion in obesity. The decreased flux index and capillary perfusion indicated vascular dysfunction. So HOMA-IR could be considered as a biomarker of vascular dysfunction in obese patients.
