Abstract
Sindrom Metabolik (SM) merupakan salah satu faktor resiko terjadinya Dry Eye Disease (DED). Kombinasi ekstrak teh hijau dan ekstrak kopi hijau dikatakan efektif dalam memperbaiki profil metabolik dan menurunkan penanda inflamasi sistemik, namun kandungan kafein diduga dapat menurunkan efektivitasnya. Penelitian ini adalah penelitian pertama yang meneliti pengaruh kombinasi ekstrak teh hijau dan kopi hijau dengan dan tanpa dekafeinasi terhadap parameter DED dan ekspresi CD11b+ kornea tikus model SM. Metode Studi eksperimental yang menggunakan hewan coba tikus Sprague Dawley. Induksi SM dilakukan dengan pemberian diet tinggi lemak tinggi sukrosa dan injeksi streptozotocin. Hewan coba dibagi menjadi 4 kelompok, yaitu kontrol negatif (KN), kontrol positif (KP), SM dengan kombinasi ekstrak terdekafeinasi (DC), serta SM dengan kombinasi ekstrak tanpa dekafeinasi (ND). Parameter DED yang diperiksa yaitu Tear Break Up Time dan Ocular Surface Staining menggunakan skema oxford grading scale. Ekspresi CD11b+ kornea diperiksa dengan metode immunofluorescence. Hasil Didapatkan penurunan ekspresi CD11b+ kornea yang signifikan pada kelompok DC dan ND dibandingkan kelompok KP (p=0.003; p=0.018), namun tidak didapatkan perbedaan signifikan antara kelompok DC dan ND (p=0.540). Pada parameter DED tidak didapatkan perbedaan bermakna antar masing-masing kelompok, namun secara histologi korena terdapat perbedaan antara KN dengan kelompok KP, DC dan ND. Jumlah pembuluh limfatik kornea pada kelompok KP berbeda signifikan dengan kelompok KN, DC dan ND (p= 0.000; p=0.007 dan p =0.001). Kesimpulan Pemberian kombinasi ekstrak, baik yang terdekafeinasi dan nondekafeinasi, tidak menunjukan pengaruh terhadap parameter DED namun memberikan pengaruh signifikan dalam menurunkan ekspresi CD11b+ kornea pada tikus model SM. Penurunan ekspresi CD11b+ tidak berbeda signifikan antara ekstrak yang terdekafeinasi dan nondekafeinasi.
English Abstract
Metabolic Syndrome (MS) is a risk factor for Dry Eye Disease (DED). The combination of green tea extract and green coffee extract is said to be effective in improving the metabolic profile and reducing markers of systemic inflammation, but the caffeine content is thought to reduce its effectiveness. This study is the first study to examine the effect of a combination of green tea and green coffee extracts with and without decaffeination on DED parameters and corneal CD11b+ expression in MS rats model. Method Experimental studies using Sprague Dawley rats as experimental animals. SM induction was carried out by administering a high-fat high-sucrose diet and streptozotocin injection. The experimental animals were divided into 4 groups, namely negative control (KN), positive control (KP), MS with a combination of decaffeinated extract (DC), and MS with a combination of non-decaffeinated extract (ND). The DED parameters examined were Tear Break Up Time and Ocular Surface Staining using the Oxford grading scale scheme. Corneal CD11b+ expression was examined using the immunofluorescence method. Result There was a significant decrease in corneal CD11b+ expression in the DC and ND groups compared to the KP group (p=0.003; p=0.018), but there was no significant difference between the DC and ND groups (p=0.540). In the DED parameters, there were no significant differences between each group, however, in histology of the cornea there were differences between KN and the KP, DC and ND groups. The number of corneal lymphatic vessels in the KP group was significantly different from the KN, DC and ND groups (p = 0.000; p = 0.007 and p = 0.001). Conclusion Administration of a combination of extracts, both decaffeinated and non-decaffeinated, did not show any effect on DED parameters but had a significant effect in reducing corneal CD11b+ expression in MS rats model. The decrease in CD11b+ expression was not significantly different between decaffeinated and nondecaffeinated extracts.
