Abstract

Diabetes Melitus (DM) termasuk dalam kelompok penyakit kronis, pemilihan obat antidiabetes harus mempertimbangkan efikasi dan keamanan yang baik untuk mencapai kontrol glukosa darah. Obat DPP-4 inhibitor dapat diberikan untuk pasien DM tipe 2 dalam bentuk tunggal atau kombinasi karena memiliki efikasi dan tingkat toleransi yang baik. Penyakit DM seringkali muncul dengan penyakit penyerta, sehingga pasien cenderung mendapat banyak obat dan kondisi ini berpeluang menyebabkan interaksi obat. Penelitian ini bertujuan untuk membandingkan efikasi obat DPP-4 Inhibitor tunggal dan kombinasi serta interaksi DPP-4 inhibitor dengan obat lain untuk penyakit penyertanya. Subjek penelitian ini adalah pasien rawat jalan di Rumah Sakit Kota Malang tahun 2022-2023 dengan penyakit DM tipe 2 yang mendapat obat DPP-4 inhibitor. Pengambilan data dilakukan secara retrospektif menggunakan 87 data rekam medis yang memenuhi kriteria inklusi. Analisis data menggunakan Statistical Program for Social Science (SPSS) dengan hasil penelitian yang didapatkan untuk parameter efikasi obat yakni terdapat perbedaan signifikan penurunan kadar HbA1c, GDP, GD2JPP dan terdapat perbedaan signifikan ketercapaian target terapi kadar HbA1c, namun tidak terdapat perbedaan signifikan ketercapaian target terapi kadar GDP dan GD2JPP pada pemberian terapi yang berbeda. Hasil interaksi obat menunjukkan bahwa terdapat perbedaan signifikan jenis obat DPP-4 inhibitor pada resep dengan kejadian interaksi obat. Kombinasi DPP-4 inhibitor, metformin, dan sulfonilurea menjadi obat yang paling efektif menurunkan kadar HbA1c, GDP dan GD2JPP. Pasien yang paling banyak mencapai target terapi adalah dengan obat kombinasi DPP-4 inhibitor dan acarbosa, dan kombinasi DPP-4 inhibitor, metformin menjadi obat yang paling berpotensi mengalami interaksi obat dengan obat lain untuk penyakit penyerta pasien.

English Abstract

Diabetes mellitus (DM) belongs to the group of chronic diseases, selection of antidiabetic drugs should consider good efficacy and safety to achieve blood glucose control. DPP-4 inhibitor drugs are available to type 2 DM patients in both monotherapy or combination forms as they have good efficacy and tolerance levels. DM disease often presents with comorbidities, so patients tend to receive multiple drugs and this condition is likely to cause drug interactions. This study aimed to compare the efficacy of DPP-4 inhibitors monotherapy and combination as the potential interaction of DPP-4 inhibitors with other drugs for comorbidities. The subjects of this study were outpatients in Malang City Hospital in 2022-2023 with type 2 DM who received DPP-4 inhibitor drugs. Data were collected retrospectively using medical record data of 87 patients who met the inclusion criteria. Data analysis with Statistical Program for Social Science (SPSS) with the results of the study obtained for drug efficacy parameters, namely there is a significant difference seen from the reduction parameter, there is no significant difference in the achievement of therapeutic targets between groups on HbA1c, GDP and GD2JPP levels. The drug interaction parameter found there was a significant difference in the use of DPP-4 inhibitor monotherapy and combination with the incidence of drug interactions. The combination of DPP-4 inhibitors, metformin, and sulfonylurea was the most effective drug in reducing HbA1c, GDP, and GD2JPP levels. Patients who achieved the most therapeutic targets were with the combination of DPP-4 inhibitors and acarbose. The combination of DPP-4 inhibitors and metformin was the drug most likely to experience drug interactions with another drug for patient comorbidities.
