Abstract

Pasien ICU membutuhkan asupan protein yang lebih tinggi karena katabolisme protein bersamaan dengan kondisi hipermetabolik. Ketidakseimbangan antara kebutuhan protein yang tinggi dan asupan protein yang tidak adekuat serta kondisi inflamasi meningkatkan risiko malnutrisi. Kondisi tersebut berkaitan dengan kejadian hipoalbuminemia yang mengarah pada hasil klinis yang buruk. Asupan protein dapat memperbaiki kondisi stres metabolik pasien ICU karena berperan dalam sintesis albumin, fungsi imun, keseimbangan cairan, regulasi hormon dan zat gizi yang dapat meningkatkan kadar albumin. Tujuan penelitian ini untuk mengetahui hubungan asupan protein enteral nutrition terhadap kadar albumin serum pada pasien ICU. Penelitian ini menggunakan desain cross sectional. Total subjek penelitian sebanyak 16 pasien sesuai kriteria inklusi yang diambil dengan teknik purposive sampling. Asupan protein enteral nutrition dan kadar albumin diambil dari data rekam medis pasien. Analisis hubungan dilakukan dengan uji korelasi Pearson. Hasil penelitian menunjukkan rata-rata asupan protein pada subjek penelitian sebesar 42,12 ± 9,45 gram (defisit). Sebagian besar subjek penelitian mengalami hipoalbuminemia dengan rata-rata kadar albumin sebesar 2,73 ± 0,45 g/dL. Hasil uji korelasi asupan protein enteral nutrition dengan kadar albumin menunjukkan adanya hubungan yang signifikan (p=0,016). Penelitian ini menyimpulkan bahwa asupan protein adekuat melalui enteral nutrition dapat memperbaiki kadar albumin serum pasien ICU.

English Abstract

ICU patients require higher protein intake due to protein catabolism and hypermetabolic conditions. The imbalance between high protein needs and inadequate protein intake, as well as inflammatory conditions, increases the risk of malnutrition. It is linked to the prevalence of hypoalbuminemia which to poor clinical outcomes. Protein intake can improve the metabolic stress condition of ICU patients as it plays a role in albumin synthesis, immune function, fluid balance, hormone regulation, and nutrients, which can increase albumin levels. This study used a cross-sectional design to determine the relationship between enteral nutrition protein intake and serum albumin levels in ICU patients. The study subjects were chosen by purposive sampling and 16 subjects were enrolled based on the inclusion criteria. Enteral nutrition protein intake and albumin levels were collected from the patient's medical record. The results show that the mean protein intake in the study subjects was 42.12 ± 9.45 grams (deficit). Most of the study subjects were hypoalbuminemia with an average albumin level of 2.73 ± 0.45 g/dL. Correlation test results of enteral nutrition protein intake with albumin levels have a significant relationship (p=0.016). This study concludes that adequate protein intake through enteral nutrition can improve serum albumin levels in ICU patients.
