Sensitivitas dan Spesifisitas Uji Fungsi Parameter Performa Fisik dalam Diagnosis Sarkopenia pada Pasien Gagal Jantung Kronis dengan Disfungsi Sistolik

Abstract

Latar belakang: Sarkopenia menurunkan kualitas hidup pasien gagal jatung kronis dan dapat dideteksi melalui uji performa fisik. Tujuan penelitian ini adalah untuk mengetahui kemampuan berbagai uji performa fisik dalam menegakkan diagnosis sarkopenia populasi tersebut. Metode: Studi potong lintang telah dilakukan. Uji performa fisik yang diuji meliputi uji 5-times sit to stand (5T-STS), kecepatan jalan, short physical performance battery (SPPB), timed up and go (TUG). Analisis kurva receiver operating characteristic (ROC) dilakukan untuk menilai performa diagnostik masing-masing uji performa fisik tersebut. Hasil: Sebanyak 32 pasien gagal jantung dengan fraksi ejeksi ventrikel kiri <50% dengan pengobatan optimal selama ≥ 3 bulan dilibatkan dalam penelitian ini. Prevalensi sarkopenia sebesar 21.9%. Uji TUG memiliki performa diagnostik yang sangat baik (area under the curve [AUC] = 0,8057; 95% confidence interval [CI] = 0,6271 hingga 0,9843) dan uji 5T-STS memiliki performa dignostik yang baik (AUC =0,7657; 95% CI = 0,5860 hingga 0,9454). Performa diagnostik yang tidak memuaskan dan cukup memuaskan masing-masing diperlihatkan oleh uji kecepatan jalan (AUC = 0,5686; 95% CI = 0,3114 hingga 0,8257) dan SPPB (AUC = 0,6600; 95% CI = 0,4570 hingga 0,8630). Uji TUG dengan titik potong 8,64 detik memiliki sensitivitas 85.7% dan spesifitas 84.0% dalam mendeteksi sarkopenia. Kesimpulan: Uji TUG memiliki perfoma diagnostik yang terbaik dalam mendeteksi sarkopenia pada pasien gagal jantung kronis dengan disfungsi sistolik.

English Abstract

Background: Sarcopenia significantly impairs the quality of life in patients with chronic heart failure and can be detected through physical performance tests. This study aimed to evaluate the diagnostic capabilities of various physical performance tests in confirming the diagnosis of sarcopenia in this population. Methods: A cross-sectional study was conducted. Physical performance tests included the 5-times sit-to-stand (5T-STS) test, gait speed, Short Physical Performance Battery (SPPB), and Timed Up and Go (TUG). Receiver Operating Characteristic (ROC) curve analysis was performed to assess the diagnostic performance of each physical performance test. Results: A total of 32 patients with chronic heart failure, with a left ventricular ejection fraction of <50%, undergoing optimal treatment for ≥3 months, were included in this study. The prevalence of sarcopenia was 21.9%. The TUG test demonstrated excellent diagnostic performance (area under the curve [AUC] = 0.8057; 95% confidence interval [CI] = 0.6271 to 0.9843), while the 5T-STS test showed good diagnostic performance (AUC = 0.7657; 95% CI = 0.5860 to 0.9454). Gait speed exhibited unsatisfactory diagnostic performance (AUC = 0.5686; 95% CI = 0.3114 to 0.8257), and SPPB showed fair diagnostic performance (AUC = 0.6600; 95% CI = 0.4570 to 0.8630). With a cutoff point of 8.64 seconds, the TUG test demonstrated a sensitivity of 85.7% and specificity of 84.0% in detecting sarcopenia. Conclusion: The TUG test received the best diagnostic performance in detecting sarcopenia in chronic heart failure patients with systolic dysfunction.
