Abstract

Limfoma Non-Hodgkin (LNH) merupakan penyakit ganas yang berasal dari sel sistem imun yang menunjukkan gejala pembengkakan kelenjar getah bening atau pembentukan tumor padat. Menurut laporan Global Cancer Observatory oleh International Agency for Reserch on Cancer tahun 2020, penambahan kasus LNH mencapai 544.352 ribu kasus (peringkat ke-11) dari penambahan kasus kanker terbanyak di dunia. LNH menduduki peringkat ke-7 sebagai kasus kanker baru terbanyak di Indonesia dengan jumlah 16.125 kasus, angka mortalitasnya peringkat ke-9 dengan 9.024 kasus kematian. Tujuan penelitian ini untuk mengetahui hubungan antara variasi implementasi guideline terapi (variasi regimen, siklus dan durasi terapi) terhadap luaran pasca terapi dan luaran tiga tahun pasien LNH. Penelitian ini dilakukan dengan pendekatan kohort retrospektif dari data rekam medis tahun 2020 di RSSA pada 82 pasien sesuai kriteria. Variabel penelitian adalah variasi implementasi guideline (regimen, siklus dan durasi terapi) sebagai variabel bebas serta luaran pasca terapi dan luaran tiga tahun sebagai variabel terikat. Hasil uji komparatif chi square antara variasi regimen dengan luaran pasca terapi didapatkan p value=0,199 dan luaran tiga tahun dengan p value=0,255 berarti keduanya tidak terdapat hubungan signifikan. Hasil uji chi square variasi siklus dan luaran pasca terapi didapatkan p value=0,094 dan luaran tiga tahun dengan p value=0,440 berarti keduanya tidak terdapat hubungan signifikan. Hasil uji Kruskal Wallis pada rerata durasi terapi dengan luaran pasca terapi didapatkan p value=0,504 berarti tidak ada hubungan signifikan. Hasil uji Mann-Whitney pada rerata variasi durasi terapi kelompok pasien yang hidup dan kelompok pasien yang meninggal (luaran tiga tahun) didapatkan p value=0,722 berarti tidak terdapat hubungan signifikan.

English Abstract

Non-Hodgkin Lymphoma (NHL) is a malignancy originating from the immune system cells, characterized by symptoms such as swollen lymph nodes or the formation of solid tumors. According to the Global Cancer Observatory report by the International Agency for Research on Cancer in 2020, the increase in NHL cases reached 544,352 thousand cases (ranking 11th) among the highest increases in cancer cases worldwide. NHL ranks 7th as the most common new cancer case in Indonesia with a total of 16,125 cases, and its mortality rate is ranked 9th with 9,024 death cases. This research aims to investigate the relationship between the variations in the implementation of therapy guidelines (regimen, cycles, and duration) to the post-therapy outcomes and three-year outcomes of NHL patients. This study employed a retrospective cohort approach using medical record data from 2020 at RSSA, involving 82 patients meeting the criteria. The research variables include variations in the implementation of guidelines (regimen, cycles, and duration) as independent variables, with post-therapy and three-year outcomes as the dependent variables. The results of comparative chi-square test between regimen variations and post-therapy (p=0,199) and three-year outcomes (p=0,255), indicates no significant relationship. The chi-square test results for cycle variations and post-therapy (p=0,094), and three-year outcomes (p=0,440), indicates no significant relationship. The Kruskal-Wallis test on the average therapy duration and post-therapy (p=0,504), indicates no significant relationship. The Mann-Whitney test on the average duration of therapy variations between the group of surviving patients and the group of deceased patients (three-year outcomes) (p=0,722), indicates no significant relationship
