Abstract

Latar Belakang: Lupus eritematosus sitemik (LES) merupakan penyakit autoimun yang mempunyai manifestasi beragam pada berbagai organ. Salah satu manifestasi penyakit LES adalah lupus nefritis (LN) yang sering menyebabkan gagal ginjal dan kematian. MicroRNA 146a memiliki peran penting dalam mengatur ekspresi gen inflamasi dan banyak ditemukan di glomerulus. Sitokin memainkan peran penting dalam patogenesis LN dan mungkin berguna untuk biomarker LN. Penelitian ini bertujuan untuk mengevaluasi ekspresi miRNA 146a dan TNF-like Weak Inducer of Apoptosis (TWEAK) urine untuk mendeteksi LN karena bukan prosedur invasif. Prosedur standar emas untuk mendiagnosis LN memerlukan biopsi ginjal. Namun, prosedurnya invasif, berbiaya tinggi, dan memakan waktu. Oleh karena itu, biomarker dari urine diperlukan untuk diagnosis LN. Metode: Penelitian yang dilakukan adalah cross-sectional. Total subjek berjumlah 69 orang, terdiri dari lupus nefritis 29 orang, lupus tanpa nefritis 28 orang, dan kontrol sehat 12 orang. Ekspresi MiRNA 146a ditentukan dengan rtPCR dan kadar TWEAK ditentukan dengan metode ELISA. Analisis statistik menggunakan korelasi Kruskal-Wallis dan Spearman. Hasil: Penelitian ini menunjukkan terdapat perbedaan kadar TWEAK urine yang signifikan antara lupus nefritis dan LES tanpa nefritis (p=0,005), namun tidak terdapat perbedaan bermakna antara ekspresi miRNA 146a antara lupus nefritis dan LES tanpa nefritis (p=0,412) . Tidak terdapat hubungan bermakna antara ekspresi miRNA 146a dengan kadar TWEAK (p=0,837).

English Abstract

Background: Systemic lupus erythematosus (SLE) is an autoimmune disease that has various manifestations in various organs. One of the manifestations of SLE is lupus nephritis (LN), which often causes kidney failure and death. MicroRNA 146a has an important role in regulating inflammatory gene expression and is widely found in the glomerulus. Cytokines play an essential role in the pathogenesis of LN and might be helpful for LN biomarkers. This study aimed to evaluate miRNA 146a expression and urine TNF-like weak inducer of apoptosis (TWEAK) for detecting LN since this is not an invasive procedure. The gold standard procedure for diagnosing LN needs a biopsy of the kidney. However, the procedure is invasive, high cost, and takes time. Thus, a biomarker from urine is needed for diagnosis of LN. Method: This research conducted was cross-sectional. The total subject were 69, consisting of 29 lupus nephritis, 28 SLE without nephritis and 12 healthy control. MiRNA 146a expression were determined by real time PCR and TWEAK levels were determined by ELISA method. Statistical analysis using Kruskal–Wallis and Spearman correlation. Results: This study showed a signifcant diference in TWEAK levels between lupus nephritis and SLE without nephritis (p=0.005), but no significant difference between miRNA 146a expression between lupus nephritis and SLE without nephritis (p=0.412). There were no significant correlations between miRNA 146a expression and TWEAK level (p=0.837).
