Abstract

Coronavirus Diseases-19 (COVID-19) merupakan penyakit pneumonia atipikal baru yang muncul pertama kali di Kota Wuhan, Provinsi Hubei, China pada akhir tahun 2019 lalu. COVID-19 terjadi akibat infeksi severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) yang menular melalui droplet. Diagnosis COVID-19 ditegakkan melalui pemeriksaan laboratorium dengan metode deteksi molekuler SARS-CoV-2 seperti RT-qPCR. Infeksi COVID-19 dibagi menjadi 5 kelompok derajat keparahan berdasarkan manifestasi klinisnya, yaitu asimtomatik, ringan, sedang, berat, dan kritis. Pada pasien COVID-19 umumnya terjadi penurunan nilai limfosit dalam darah atau limfopenia. Penelitian ini bertujuan untuk mengetahui hubungan kadar ALC terhadap derajat keparahan pasien COVID-19 menggunakan desain observasional analitik dengan metode cross-sectional. Status derajat keparahan dan kadar ALC pasien didapatkan dari data rekam medis pasien COVID-19 pada bulan juli-september 2021 di IGD IGD RSUD Dr. Saiful Anwar Malang. Digunakan uji statistik Kolmogrov Smirnov untuk uji normalitas mengetahui distribusi data. Pada hasil uji normalitas didapatkan data terdistribusi tidak normal (p<0.05). Digunakan uji statistik Mann-Whitney untuk uji komparatif mengetahui perbedaan yang signifikan. Pada hasil uji komparatif didapatkan perbedaan bermakna kadar ALC antara pasien COVID-19 derajat keparahan mild�moderate dan severe-critical (p<0,05) dengan nilai AUC 0,578 dan OR 2,074 sehingga disimpulkan ALC dapat menjadi biomarker derajat keparahan COVID-19 tapi belum bisa menjadi biomarker mutlak.

English Abstract

Coronavirus Diseases-19 (COVID-19) is a new atypical pneumonia disease that first appeared in Wuhan City, Hubei Province, China at the end of 2019. COVID-19 occurs due to infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) which is transmitted through droplets. The diagnosis of COVID-19 is made through laboratory examination using SARS-CoV-2 molecular detection methods such as RT-qPCR. COVID-19 infection is divided into 5 severity groups based on clinical manifestations, namely asymptomatic, mild, moderate, severe and critical. In COVID-19 patients generally there is a decrease in the value of lymphocytes in the blood or lymphopenia. This study aims to determine the relationship between ALC levels and the severity of COVID-19 patients using an analytical observational design with methodscross-sectional. The patient's severity status and ALC levels were obtained from medical record data of COVID-19 patients in July-September 2021 in the Emergency Room of Dr. RSUD. Saiful Anwar Malang. The Kolmogrov Smirnov statistical test was used to test normality to determine the distribution of the data. In the normality test results, it was found that the data was not normally distributed (p<0.05). The Mann-Whitney statistical test was used for comparative tests to determine significant differences. Comparative test results showed significant differences in ALC levels between COVID-19 patients with degrees of severity mild-moderate and severe-critical (p<0.05) with an AUC value of 0.578 and OR 2.074 so it can be concluded that ALC can be biomarker the severity of COVID-19 but it cannot yet become biomarker absolute
