Abstract

Hematemesis dan melena adalah tanda-tanda pendarahan gastrointestinal bagian atas. Hematemesis didefinisikan sebagai muntah darah atau bekuan darah, sedangkan melena didefinisikan sebagai tinja yang gelap dan beraroma dengan bau menyengat yang khas. Pendarahan saluran cerna atas merupakan keadaan yang mengancam jiwa. Maka dari itu dibutuhkan sebuah tools untuk mengidentifikasi pasien yang beresiko tinggi mengalami pendarahan, terutama pada pasien cirrhosis hepatis. Skor IMPROVE (The International Medical Prevention Registry on Venous Thromboembolism) adalah model untuk memprediksi risiko perdarahan untuk pasien rawat inap yang memiliki risiko VTE. Skor IMPROVE berpotensi digunakan sebagai prediktor kejadian perdarahan saluran cerna atas pada pasien cirrhosis hepatis. Penelitian ini bertujuan untuk membandingkan derajat skor IMPROVE dengan kejadian perdarahan saluran cerna atas pada pasien dengan cirrhosis hepatis. Penelitian ini merupakan penelitian cross-section pada pasien cirrhosis hepatis dengan menggunakan uji chi square. Subjek pada penelitian ini merupakan pasien terkonfirmasi cirrhosis hepatis di RSUD Dr. Saiful Anwar Malang pada tahun 2022 – 2023 berjumlah 100 pasien, 50 pasien untuk kelompok yang mengalami perdarahan dan 50 pasien untuk kelompok yang tidak mengalami perdarahan. Hasil penelitian ini yaitu didapatkan p value sebesar 0.000, yang menunjukkan bahwa terdapat perbandingan yang signifikan antara kejadian perdarahan saluran cerna atas dengan skor IMPROVE pada pasien cirrhosis hepatis. Selain itu hasil uji risk estimate menunjukkan pasien dengan skor IMPROVE risiko tinggi memiliki peluang mengalami perdarahan saluran cerna atas sebanyak 32.250 kali dibandingkan dengan pasien dengan skor IMPROVE dengan risiko rendah.

English Abstract

Hematemesis and melena are signs of upper gastrointestinal bleeding (UGIB). Hematemesis is characterized as vomiting blood with brownish color, while melena is characterized as dark, scented stools with pungent odor. Upper gastrointestinal bleeding (UGIB) is a life-threatening condition. Therefore, a tool is needed to identify patients who are at high risk of bleeding, especially in patients with cirrhosis hepatis. IMPROVE (The International Medical Prevention Registry on Venous Thromboembolism) bleeding score is a model to predict bleeding risk for hospitalized patients at risk of VTE. The IMPROVE score has the potential to be used as a predictor of upper gastrointestinal bleeding in patients with cirrhosis hepatis. This study aims to compare the degree of IMPROVE score with the incidence of upper gastrointestinal bleeding in patients with cirrhosis hepatis. This research is a cross-section study and using chi square comparison method. The subjects in this study were patients with confirmed cirrhosis hepatis at RSUD Dr. Saiful Anwar Malang in 2022 – 2023 numbered 100 patients, 50 patients in the group who experienced bleeding and 50 patients in the group who did not experience bleeding. The results of this study are a p value of 0.000, which shows that there is a significancy between the incidence of upper gastrointestinal bleeding and the IMPROVE score cirrhosis hepatis patients. In addition, the results of the risk estimate test show that a patient with high risk IMPROVE score has a 32,250 times chance of experiencing upper gastrointestinal bleeding compared to patients with a low risk IMPROVE score.
