Abstract

Latar Belakang: Cervical Radiculopathy merupakan salah satu kondisi kesehatan yang menjadi penyebab utama disabilitas di seluruh dunia. Dalam penegakan diagnosisnya, diperlukan kesinambungan antara anemnesis, pemeriksaan fisik, dan pemeriksaan penunjang. Umumnya, pasien akan mengalami gangguan motorik sesuai lesi pada radiks yang mempersarafi otot. Namun, ketersediaan fasilitas penegakan diagnosis EMG di FKTP cenderung minim dan terkadang memberikan hasil false positif maupun negative. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan antara Compound Muscle Action Potential (CMAP) dengan derajat kekuatan motorik pada EMG pasien Cervical Radiculopathy. Metode: Penelitian observasional analitik retrospektif dengan analisis desain studi cross sectional menggunakan data rekam medis pasien Cervical Radiculopathy di RSSA Malang. Penelitian ini menggunakan metode kuota sampling untuk mendapatkan 83 sampel yang memenuhi kriteria inklusi. Amplitudo, Latensi, dan KHS CMAP di analisis dengan uji Spearman dan Pearson untuk mengetahui korelasinya terhadap kekuatan motorik. Uji Mann Whitney dilakukan untuk melihat perbandingan CMAP pasien dengan kekuatan motorik normal dan tidak normal. Hasil: Hasil korelasi menunjukkan amplitudo, latensi, dan KHS CMAP pasien tidak signifikan dengan derajat kekuatan motorik (p 0.78, p 0.76, dan p 0.54 p>0.05) pada uji n. medianus kanan. Namun, uji komparasi menunjukkan signifikansi antara amplitud0 dan latensi CMAP n. medianus kanan pada pasien dengan kekuatan motorik normal dibanding tidak normal (p 0.00, p 0.00; p <0.05). Kesimpulan: Tidak terdapat hubungan antara CMAP dengan derajat kekuatan motorilk pasien Cervical Radiculopathy. Namun, terdapat perbedaan yang signifikan antara CMAP pada pasien dengan kekuatan motorik normal dan tidak normal.

English Abstract

Background: Cervical Radiculopathy is a health condition that is commonly made disability throughout the world. In the diagnostic approach, continuity between the history of the patient, physical examination, and supporting examinations is required. Generally, patients will experience motor disorders according to lesions in the roots that innervate the muscles. However, the availability of EMG diagnosis facilities at FKTP tends to be minimal and sometimes produces false positive or negative results. Objective: This study aims to determine the relationship between Compound Muscle Action Potential (CMAP) and the degree of motor strength in the EMG of Cervical Radiculopathy patients. Methods: Retrospective analytical observational study with cross-sectional study design analysis using medical record data from Cervical Radiculopathy patients at RSSA Malang. This research used quota sampling to obtain 83 samples that met the inclusion criteria. Amplitude, Latency, and KHS CMAP were analyzed using Spearman and Pearson tests to determine their correlation with motor strength. The Mann-Whitney test was carried out to compare CMAP in patients with normal and abnormal motor strength. Results: The correlation results showed that the patient's CMAP amplitude, latency, and KHS were insignificant with the degree of motor strength (p 0.78, p 0.76, and p 0.54 p>0.05) in the right medians test. However, the comparison test showed significance between CMAP amplitude and latency ) of right medians in patients with normal motor strength compared to abnormal (p 0.00, p 0.00; p <0.05). Conclusion: There is no relationship between CMAP and the degree of motor strength in Cervical Radiculopathy patients. However, there were significant differences between CMAP in patients with normal and abnormal motor strength.
