Abstract

Tujuan : mengetahui hubungan antara Asymmetric Dimethylarginine (ADMA) dengan densitas perfusi dan ketebalan Retinal Nerve Fiber Layer (RNFL) papil saraf optic pada oebsitas. Metode : Penelitian dengan desain observasional kasus kontrol yang di lakukan pada bulan September 2022 – Juli 2023 di poli Mata dan laboratorium sentral RSUD dr Saiful Anwar Malang. Melibatkan pada 34 orang sebagai kontrol dan 40 orang sebagai kelompok obesitas dengan rentang usia 25 - 54 tahun, dan tajam penglihatan dengan koreksi lebih baik dari 0,3 logMAR. Dilakukan pemeriksaan ELISA untuk mengetahui kadar ADMA dalam plasma, pemeriksaan Optical Coherence Tomography (OCT) dan Optical Coherence Tomography Angiography (OCTA) untuk mengetahui ketebalan RNFL dan densitas perfusi papil saraf optik. Data dianalisa dengan SPSS versi 23. Hasil : Korelasi yang signifikan hanya pada hubungan antara kadar ADMA dengan densitas perfusi regio full (p<0.05). Hubungan tersebut sebesar 32.7% (r= -0.327) Kesimpulan : Tingginya kadar ADMA maka semakin menurunnya densitas perfusi regio full dan sebaliknya. Mekanisme peningkatan ADMA dan perubahan ketebalan RNFL dapat diperkirakan merupakan suatu proses yang berhubungan namun hal ini berkebalikan dengan temuan pada studi kami yaitu tidak terdapat korelasi antara kadar ADMA dan penebalan RNFL.

English Abstract

Objective: to assess correlation of asymmetric dimethylarginine (ADMA) on perfusion density and optic retinal nerve fiber layer (RNFL) thickness in obesity. Methods: This observational case-control study was conducted from September 2022 to July 2023 at the ophthalmology outpatient clinic and central laboratory of Dr. Saiful Anwar Hospital, Malang. The study involved 34 individuals as controls and 40 individuals in the obesity group, aged between 25 and 54 years, with corrected visual acuity better than 0.3 logMAR. ELISA assays were performed to determine ADMA levels in plasma, and Optical Coherence Tomography (OCT) and Optical Coherence Tomography Angiography (OCTA) examinations were conducted to assess RNFL thickness and optic nerve head perfusion density. Data were analyzed using SPSS version 23. Results: A significant correlation was observed only in between ADMA levels and full-region perfusion density, with a correlation coefficient of 32.7% (r= -0.327). There is no significant correlation between ADMA levels and RNFL thickness. Conclusion: Elevated ADMA levels are associated with a decrease in full-region perfusion density, and vice versa. The mechanism behind increase ADMA and changes in RNFL thickness is presumed to be a related process. However, this contradicts the findings in our study, where no correlation was found between ADMA levels and RNFL thickening.
