Abstract
Stroke menjadi penyebab kematian terbanyak ketiga di dunia setelah penyakit jantung koroner. Pada tahun 2018 WHO melaporkan bahwa angka kematian akibat stroke setiap tahunnya terus meningkat, yaitu lebih dari 137.000 orang pertahun. Dalam menegakkan diagnosis stroke, dibutuhkan alat penunjang CT (Computerized Tomography) scan kepala untuk membedakan tipe stroke (iskemik atau perdarahan) secara definitif. Namun, tidak semua rumah sakit memiliki fasilitas layanan tersebut. Oleh karena itu, diperlukan suatu alat diagnostik klinis berupa sistem skoring sederhana untuk membedakan stroke hemoragik atau stroke iskemik dengan menggunakan Siriraj Stroke Score (SSS). SSS banyak digunakan di Thailand dan lebih mudah digunakan, bahkan sudah divalidasi di Nigeria, Taiwan dan Malaysia. Penelitian ini bertujuan untuk mengetahui perbedaaan Siriraj Stroke Score pada pasien stroke iskemik dan stroke hemoragik di Instalasi Gawat Darurat RSUD Dr. Saiful Anwar Malang. Penelitian ini menggunakan desain deskriptif dengan pendekatan cross sectional. Populasi dalam penelitian ini sebanyak 60 pasien stroke di IGD RSSA selama periode 24 Juni sampai 19 Agustus 2023. Pengambilan sampel menggunakan purposive sampling dengan sampel sejumlah 33 pasien suspek stroke dan didapatkan bahwa sebagian besar hasil SSS adalah < -1 sebesar 21 pasien atau 64% (stroke iskemik), diikuti dengan skor > 1 sebesar 33% atau 11 pasien (stroke hemoragik) dan 1 pasien (3%) dengan hasil SSS 1 s/d -1 (meragukan). Uji beda dua variabel menggunakan uji independent sample t-test dengan hasil p-value 0,001<0,05, sehingga menunjukkan bahwa ada perbedaan rata-rata yang signifikan antara hasil Siriraj Stroke Score pada stroke iskemik dan stroke hemoragik. Penerapan Siriraj Stroke Score untuk membedakan jenis stroke sangat diperlukan terutama dalam pelayanan di rumah sakit agar dapat dilakukan tatalaksana yang tepat dan cepat dalam pemberian trombolitik dan tercapainya golden hour pada pasien stroke.
English Abstract
The third leading cause of death in the world after coronary heart disease was stroke. In 2018, World Health Organization (WHO) reported that the annual death toll due to stroke continued to rise, with more than 137,000 people each year. In establishing a stroke diagnosis, a supportive tool such as a head Computerized Tomography (CT) scan was needed to definitively differentiate between stroke types (ischemic and hemorrhagic). However, not all hospitals had such service facilities. Therefore, a clinical diagnostic tool was needed in the form of a simple scoring system to distinguish hemorrhagic stroke from ischemic stroke using the Siriraj Stroke Score (SSS). SSS was widely used in Thailand and proved to be user-friendly. It had even been validated in Nigeria, Taiwan, and Malaysia. This study aimed to determine the difference in Siriraj Stroke Scores between ischemic and hemorrhagic stroke patients at the Emergency Department of RSUD Dr. Saiful Anwar Malang. This research used a descriptive design with a cross-sectional approach. The population in this study comprised 60 stroke patients in Emergency Department of RSUD Dr. Saiful Anwar during the period from June 24 to August 19, 2023. Pusposive sampling used with a sample size of 33 suspected stroke patients, and it was found that the majority of SSS results were < -1 in 21 patients or 64% (ischemic stroke), followed by scores > 1 in 33% or 11 patients (hemorrhagic stroke), and 1 patient (3%) with SSS results between 1 and -1 (doubtful). The difference between two variables was tested using an independent sample t-test with a p-value of 0.001<0,05, indicating a significant average difference in Siriraj Stroke Scores between ischemic and hemorrhagic strokes. The implementation of the Siriraj Stroke Score to differentiate between stroke types was crucial, especially in hospital services. This allowed for the administration of prompt and appropriate management, including the timely provision of thrombolytics, aiming to achieve the golden hour of stroke.
