Abstract
COVID-19 merupakan penyakit yang mewabah dan menjadi pandemi mulai Desember 2019 hingga sekarang. Tingkat keparahan dari COVID-19 sendiri ditandai seiring dengan keparahan inflamasi yang disebabkan SARS-CoV-2. Oleh karena itu, penggunaan biomarker yang efektif dapat membantu proses skrining, penanganan klinis, dan pencegahan komplikasi. Biomarker NLR dan CRP merupakan biomarker yang mudah diperiksa dan dapat digunakan untuk memprediksi prognosis dari pasien COVID-19. Penelitian ini bertujuan mengetahui performa NLR dan CRP dalam memprediksi mortalitas pasien COVID-19 dengan komorbid di RSSA Malang. Penelitian Cross-Sectional Retrospective ini mengambil sampel dengan cara consecutive sampling dari rekam medis mulai November 2020 - Maret 2022. Hasil analisis data biomarker terhadap mortalitas didapatkan hasil uji beda Mann-Whitney signifikan pada NLR (p<0,05) dan CRP (p<0,05). Hasil Area Under Curve (AUC) NLR dari ketiga gelombang yaitu 0,739. Sedangkan, pada CRP 0,723. Dilakukan analisis penentuan Cut-Off NLR pada ketiga gelombang didapatkan 5,72 dengan sensitivitas 69% dan spesifisitas 67%. Sedangkan, pada CRP didapatkan Cut-Off 6,15 mg/dL dengan sensitivitas 68% dan spesifisitas 68%. Kesimpulan penelitian ini biomarker NLR dan CRP dapat digunakan sebagai prediktor mortalitas pasien COVID-19 dengan komorbid pada gelombang pertama hingga ketiga.
English Abstract
COVID-19 is a disease that has spread and become a pandemic from December 2019 until now. The severity of COVID-19 itself is characterized by the severity of the inflammation caused by SARS-CoV-2. Therefore, the use of effective biomarkers can help the screening process, clinical management, and prevention of complications. NLR and CRP biomarkers are biomarkers that are easy to check and can be used to predict the prognosis of COVID-19 patients. This study aims to determine the performance of NLR and CRP in predicting mortality in COVID-19 patients with comorbidities at RSSA Malang. This Cross-Sectional Retrospective study took samples by consecutive sampling from medical records from November 2020 - March 2022. The results of the analysis of biomarker data on mortality showed that the Mann-Whitney difference test was significant for NLR (p<0.05) and CRP (p<0.05). 0.05). The NLR Area Under Curve (AUC) result from the three waves is 0.739. Meanwhile, the CRP is 0.723.
