Abstract

Pasien dengan kondisi sakit kritis sangat berhubungan dengan pemberian terapi cairan. Terapi cairan adalah salah satu terapi sangat menentukan keberhasilan penanganan pasien kritis. Hipervolemia sering dikaitkan dengan tingginya morbiditas dan mortalitas pasien sakit kritis yang berefek paling sering pada 2 sistem organ, yaitu paru-paru dan ginjal. Pada paru akan menyebabkan gangguan pertukaran gas, penurunan komplain paru, dan peningkatan usaha napas sehingga sering menyebabkan acute respiratory distress syndrome (ARDS). Tujuan penelitian ini adalah Mengetahui hubungan Antara kondisi hipervolemia dengan kejadian mortalitas pada pasien anak kritis. Metode penelitianya menggunakan pendekatan deskriptif kuantitatif. Populasi dalam penelitian ini adalah seluruh pasien di PICU, sedangkan sampelnya diambil secara consecutive sampling selama waktu 2 bulan dari februari-maret 2023 didapatkan 73 pasien dimana 34 pasien mati dan 39 pasien hidup. Hasil penelitian menunjukan pasien yang mengalami kematian dengan Hipervolemia di PICU sebanyak (55,8%) berada pada usia > 1 tahun sbanyak (58%) , sebagian besar (58%) berjenis kelamin laki-laki. analisa menggunakaniuji Shaperman Rho didapat nilai p 0,000. Karena nilai P <I 0,005 maka dapat disimpulkan bahwa terdapat hubungan yang signifikan antaraiHipervolemia dengan angka kejadianimortalitas. Diharapkanipeneliti selanjutnya untuk meneliti tentang faktor apa saja yang paling mempengaruhi kejadian mortalitas di ICU sehingga diketahui dengan jelas bagaimana cara dalam menurunkan angka mortalitas.

English Abstract

Patients with critically ill conditions are closely related to the provision of fluid therapy. Fluid therapy is one of the therapies that really determines the success of treating critical patients. Hypervolemia is often associated with high morbidity and mortality in critically ill patients with effects most often on 2 organ systems, namely the lungs and kidneys. In the lungs, it will cause disruption of gas exchange, decreased pulmonary complaints, and increased respiratory effort, often causing acute respiratory distress syndrome (ARDS). The aim of this research is to determine the relationship between hypervolemia and mortality in critical pediatric patients. The research method uses a quantitative descriptive approach. The population in this study was all patients in the PICU, while the sample was taken by consecutive sampling over a period of 2 months from February-March 2023, resulting in 73 patients, of which 34 patients died and 39 patients lived. The results of the study showed that as many as (55.8%) of the patients who experienced death with hypervolemia in the PICU were aged > 1 year, the majority (58%) were male. analysis using the Shaperman Rho test obtained a p value of 0.000. Because the P value is <0.005, it can be concluded that there is a significant relationship between hypervolemia and the incidence of mortality. It is hoped that future researchers will examine what factors most influence the incidence of mortality in the ICU so that it is clear how to reduce the mortality rate.
