Abstract

Pendahuluan: Lupus Eritematosus Sistemik (LES) adalah penyakit autoimun sistemik dengan manifestasi klinis yang beragam. Kehadiran immunosenescence dini dalam LES memiliki dampak pada peningkatan morbiditas dan mortalitas pasien. Beberapa penelitian telah digunakan untuk menentukan perkembangan LES, seperti skor SLEDAI dan kadar cCD28 dan cCD80. Tujuan: Penelitian ini bertujuan untuk menentukan korelasi antara kadar cCD28 dan cCD80 dan skor SLEDAI pada pasien LES. Metode: Penelitian desain cross sectional pada populasi pasien wanita muda yang didiagnosis dengan LES berdasarkan kriteria klasifikasi SLICC 2012 yang mencari perawatan di Poliklinik Reumatologi RSSA Malang dalam periode 6 bulan. Pasien dengan penyakit ginjal parah dan infeksi dikecualikan dari populasi sampel untuk analisis lebih lanjut. Tingkat plasma sCD28 dan sCD80 dalam darah diukur, dan kemudian skor SLEDAI dinilai. Korelasi antara variabel dianalisis menggunakan uji Spearman dengan SPSS 23.0.. Hasil: Karakteristik subjek penelitian menunjukkan bahwa rata-rata pasien lebih dari 30 tahun (31,8 ± 11,0 tahun). Berdasarkan tingkat aktivitas penyakit, 11 orang (31%) mengalami penyakit ringan, 24 orang (69%) mengalami penyakit sedang, dan tidak ada yang mengalami penyakit berat. Pasien LES rata-rata memiliki tingkat sCD28 dan sCD80 sebesar 24,93 dan 27,41. Hasil uji Spearman menunjukkan bahwa tingkat sCD28 memiliki korelasi signifikan dengan skor SLEDAI [R=0,364; p=0,031 (p<0,05)], sementara tingkat sCD80 tidak berkorelasi secara signifikan dengan skor SLEDAI [R=-0,048; p=0,786 (p>0,05)]. Kesimpulan: Kadar sCD28 berkorelasi dengan skor SLEDAI, sedangkan kadar sCD80 tidak berkorelasi dengan skor SLEDAI.

English Abstract

Introduction: Systemic Lupus Erythematosus (SLE) is a systemic autoimmune disease with diverse clinical manifestations. The presence of early immunosenescence in SLE has an impact on increasing patient morbidity and mortality. Several studies have been used to determine the development of SLE, such as the SLEDAI score and soluble costimulatory levels (CD28 and CD80). Objective: This research aims to determine the relationship between soluble costimulatory levels and SLEDAI scores in SLE patients. Methods: Cross-sectional study on this patient population young woman diagnosed with SLE based on the 2012 SLICC classification criteria who sought treatment at the RSSA Malang Rheumatology Polyclinic within a period of 6 months. Patients with severe kidney disease and infections excluded from the sample population for further analysis. Plasma levels of sCD28 and sCD80 in the blood were measured and then the SLEDAI score was assessed. Correlation between variables was analyzed using the Spearman test with SPSS 23.0. Results: The characteristics of the research subjects showed that the average patient was more than 30 years old (31.8± 11.0 year). Based on degree of disease activity, 11 people (31%) had mild disease, 24 people (69%) had moderate disease and no serious disease. SLE patients on average have levels sCD28 and sCD80 (24.93; 27.41). Spearman test results obtained levels sCD28 had a significant correlation with SLEDAI scores [R=0.364; p=0.031(p<0.05)] while levels sCD80 did not correlate significantly with SLEDAI scores [R=-0.048; p=0.786(p>0.05)]. Conclusion: sCD28 levels has correation with SLEDAI score and sCD80 levels has no correlation with SLEDAI score.
