Abstract
Tuberkulosis resistan obat merupakan penyakit disebabkan oleh bakteri Mycobacterium tuberculosis yang resistan terhadap obat anti tuberkulosis. Terjadinya resistansi dipengaruhi oleh beberapa faktor seperti kekebalan alami yang dimiliki bakteri, ketidakpatuhan pasien selama pengobatan, dan kesalahan pemberian antibiotik selama masa terapi. Pengobatan tuberkulosis resistan obat dapat menimbulkan efek samping hematologi, salah satunya adalah leukopenia. Penelitian ini bertujuan untuk mengetahui gambaran efek samping pengobatan tuberkulosis resistan obat terhadap kejadian leukopenia dengan melihat insiden dan onset serta mengetahui hubungan karakteristik pasien meliputi jenis kelamin, usia, indeks massa tubuh, dan komorbid dengan insiden leukopenia akibat efek samping pengobatan tuberkulosis resistan obat. Penelitian ini menggunakan desain deskriptif analitik berupa cohort retrospektif. Subjek diambil dengan total sampling dan didapatkan 113 pasien yang dirawat di RSUD Dr. Saiful Anwar Malang pada september 2019-Desember 2022. Hasil penelitian menunjukkan insiden terjadinya leukopenia sebanyak 42 pasien (37,17%). Terjadi peningkatan insiden leukopenia hingga bulan ketiga, namun turun hingga akhir fase intensif. Terjadi penurunan jumlah pasien berdasarkan onset leukopenia semula 25 pasien onset cepat menjadi 17 pasien onset lambat. Analisis chi-square menunjukkan terdapat hubungan signifikan antara jenis kelamin (p-value 0,02) dengan insiden efek samping pengobatan tuberkulosis resistan obat. Namun tidak ada hubungan signifikan antara usia (p-value 0,547), indeks massa tubuh (p-value 0,769), dan komorbid (p-value 0,467).
English Abstract
Drug-resistant tuberculosis is a disease caused by bacteria Mycobacterium tuberculosis resistant to anti-tuberculosis drugs. The occurrence of resistance is influenced by several factors such as the natural immunity of bacteria, patient noncompliance during treatment, and errors in administering antibiotics during therapy. Treatment of drug-resistant tuberculosis can cause hematological side effects, one of which is leucopenia. This study aims to determine the description of the side effects of drug-resistant tuberculosis treatment on the incidence of leucopenia by looking at the incidence and onset and determining the relationship between patient characteristics including gender, age, body mass index and comorbidities with the incidence of leucopenia due to side effects of drug-resistant tuberculosis treatment. This research uses a descriptive analytic design in the form of a retrospective cohort. Subjects were taken using total sampling and it was found that 113 patients were being treated at RSUD Dr. Saiful Anwar Malang in September 2019-December 2022. The results of the study showed that the incidence of leucopenia was 42 patients (37.17%). There was an increase in the incidence of leucopenia until the third month, but decreased until the end of the intensive phase. There was a decrease in the number of patients based on the onset of leucopenia, from 25 rapid onset patients to 17 late onset patients. Analysischi-square showed that there was a significant relationship between gender (p-value 0.02) and the incidence of side effects from drug-resistant tuberculosis treatment. However, there was no significant relationship between age (p-value 0.547), body mass index (p-value 0.769), and comorbidities (p-value 0.467).
