Abstract

Defisiensi vitamin D merupakan salah satu komplikasi endokrin akibat kelebihan besi pada pasien anak dengan talasemia mayor yang memerlukan transfusi darah berulang seumur hidup sebagai pilihan terapi utama pengganti eritrosit yang terus hancur. Keadaan penumpukan besi akibat transfusi tersebut berperan penting dalam penurunan massa tulang. Selain itu, kadar vitamin D yang rendah diduga turut berpengaruh dalam mekanisme multifaktorial terjadinya osteoporosis pada pasien talasemia. Oleh karena itu, penelitian ini bertujuan untuk menganalisis hubungan antara kadar feritin dan vitamin D serum dengan kejadian osteroporosis pada pasien anak talasemia mayor khususnya yang memiliki kadar feritin serum tinggi atau hemosiderosis. Penelitian ini adalah penelitian observasional analitik yang menggunakan pendekatan cross sectional dengan populasi taget pasien anak talasemia mayor di Kota Malang. Penelitian dilakukan dengan persetujuan etik dari komisi etik RSUD Dr. Saiful Anwar Malang. Hasil analisis menggunakan metode korelasi Spearman menunjukkan hubungan negatif antara kadar feritin dan BMD (p = 0,048; r = -0,408), hubungan negatif antara kadar feritin dan kadar vitamin D (p = 0,002; r = -0,602), serta hubungan positif antara kadar vitamin D dan BMD (p = 0,002; r = 0,608). Dengan demikian, kadar vitamin D serum dapat dipertimbangkan sebagai pilihan evaluasi rutin dan penanda awal kondisi klinis pada pasien anak talasemia mayor terutama terkait osteoporosis dan gangguan pertumbuhan yang menyertainya.

English Abstract

Vitamin D deficiency is one of the endocrine complication due to iron overload in pediatric patients with thalassemia major who require repeated blood transfusions throughout their lives as the main therapy option to replace erythrocytes that continue to be destroyed. The condition of iron accumulation due to transfusion plays an important role in reducing bone mass. In addition, low vitamin D levels are thought to play a role in the multifactorial mechanism of osteoporosis in thalassemia patients. Therefore, this study aims to analyze the relationship between serum ferritin and vitamin D levels with the incidence of osteoporosis in pediatric patients with thalassemia major, especially those who have high serum ferritin levels or hemosiderosis. This research is an analytical observational study with a cross sectional approach targeting the population of thalassemia major pediatric patients in the city of Malang. The research was carried out with ethical approval from the ethics commission of General Hospital Dr. Saiful Anwar Malang. The results of analysis using the Spearman correlation method showed a negative relationship between ferritin levels and BMD (p = 0.048; r = -0.408), a negative relationship between ferritin levels and vitamin D levels (p = 0.002; r = -0.602), and a positive relationship between vitamin D levels and BMD (p = 0.002; r = 0.608). Thus, serum vitamin D levels can be considered as an option for routine evaluation and an early marker of clinical conditions in pediatric patients with thalassemia major, especially related to osteoporosis and the accompanying growth disorders.
