Abstract

Sirosis hepatis, penyakit hati kronis yang serius, menyebabkan perubahan struktur dan fungsi hati, meningkatkan risiko kematian. Sirosis hepatis dapat terjadi dengan perdarahan atau tanpa perdarahan.Penelitian terbaru fokus pada peran interleukin 6 (IL-6) dalam mempengaruhi proses pembekuan darah pada pasien sirosis. Tromboelastografi (TEG) berpotensi berguna dalam menilai risiko perdarahan pada pasien sirosis hepatis, yang dapat membantu menghindari transfusi profilaksis yang tidak tepat. Meskipun TEG menunjukkan status hemostasis yang normal pada pasien sirosis, terdapat inkonsistensi, terutama pada waktu reaksi (R time). Mengkaitkan TEG dengan kadar IL-6 bisa memberikan wawasan lebih dalam tentang keparahan sirosis hepatis dan meningkatkan pengelolaan penyakit ini. Studi ini penting untuk meningkatkan pengelolaan sirosis hepatis dan menurunkan angka mortalitas terkait. Penelitian observasional analitik cross-sectional di RSUD dr. Saiful Anwar Kota Malang ini yang melibatkan 35 pasien sirosis hepatis, bertujuan untuk mengevaluasi hubungan antara IL-6 dan parameter Tromboelastografi (TEG) pada pasien dengan dan tanpa perdarahan. Meskipun ditemukan bahwa pasien dengan perdarahan memiliki kadar IL-6 yang lebih tinggi, analisis TEG tidak menunjukkan perbedaan signifikan dalam mekanisme koagulasi antara kedua kelompok. Tidak terdapat korelasi yang bermakna antara kadar IL-6 dengan parameter TEG pada pasien sirosis hepatis dengan perdarahan. Tidak terdapat korelasi yang bermakna antara kadar IL-6 dengan parameter TEG pada pasien sirosis hepatis tanpa perdarahan. Pada pasien sirosis hepatis dengan maupun tanpa perdarahan, menunjukan adanya proses inflamasi dan koagulasi yang berjalan secara independen.

English Abstract

Hepatic cirrhosis, a serious chronic liver disease, causes structural and functional changes in the liver, increasing the risk of death. Recent research focuses on the role of interleukin 6 (IL-6) in influencing the blood coagulation process in cirrhosis patients. Thromboelastography (TEG) has potential utility in assessing bleeding risk in hepatic cirrhosis patients, which can help avoid inappropriate prophylactic transfusions. Although TEG indicates normal hemostasis status in cirrhosis patients, inconsistencies exist, especially in reaction time (R time). Linking TEG with IL-6 levels could provide deeper insights into the severity of hepatic cirrhosis and improve disease management. This study is important for enhancing the management of hepatic cirrhosis and reducing related mortality rates. This cross-sectional analytical observational study at RSUD Dr. Saiful Anwar, Kota Malang, involving 35 hepatic cirrhosis patients, aimed to evaluate the relationship between IL-6 and Thromboelastography (TEG) parameters in patients with and without bleeding. Although it was found that patients with bleeding had higher IL-6 levels, TEG analysis did not show significant differences in coagulation mechanisms between the two groups. There was no correlation between IL-6 levels and TEG parameters in hepatic cirrhosis patients with bleeding and statistically the p-value was not significant. There was no correlation between IL-6 levels and TEG parameters in hepatic cirrhosis patients without bleeding and statistically the p-value was not significant. This was the case for both hepatic cirrhosis patients with and without bleeding, indicating that inflammatory and coagulation processes operate independently.
