Abstract

Latar Belakang: Penyakit Hirschsprung adalah penyakit multigenetik kompleks, ditandai dengan tidak adanya ganglion intrinsik sel pada pleksus submucosa, pleksus mienterikus intestinal tract. Diagnosa penyakit ini secara radiologi dengan colon in loop Hirschsprung dan pemeriksaan histo patologi sebagai gold standar. Scoring Alehossein digunakan untuk menilai colon in loop Hirschsprung dibandingkan histopatologi dengan kriteria meliputi zona transisi, recto sigmoid indeks <1, kontraksi ireguler di daerah aganglionik, Ireguler mukosa spasme kolon, filling defect karena fecaloid dan tidak keluar mekonium selama 48 jam pertama kelahiran dengan nilai sensisivitas dan spesisfisitas yang baik. Tujuan Penelitian: menilai akurasi diagnosa pemeriksaan colon in loop Hirschsprung dengan scoring Alehossein di RSUD dr Saiful Anwar provinsi Jawa Timur. Metode dan Sampel Penelitian: Penelitian ini adalah observasional analitik dengan pendekatan cross sectional, dengan 40 pasien yang dilakukan colon in loop Hirschsrprung dan di konfirmasi pemeriksaan histo patologi. Hasil Peneltian dan Diskusi: Hasil penelitian didapatkan sensitifitas 96,6%, dan nilai akurasi sebesar 72,5%, spesifisitas sebesar 9,1%, nilai duga positif sebesar 73,7%, nilai duga negatif sebesar 50%. Kesimpulan: Teknik Colon in loop Hirschsprung mampu memberikan hasil positif seperti histopatologi sebagai gold standard. Namun pemeriksaan Colon in Hirschsprung's loop tidak dapat memprediksi dengan baik pada pasien yang tidak dicurigai menderita penyakit Hirschsprung.

English Abstract

Background: Hirschsprung's disease is a complex multigenetic disease, characterized by the absence of intrinsic ganglion cells in the submucosal plexus, myenteric plexus of the intestinal tract. Diagnosis of this disease is radiological with colon in Hirschsprung's loop and histo-pathological examination as the gold standard. Alehossein scoring is used to assess the colon in Hirschsprung's loop compared to histopathology with criteria including the transition zone, recto igmoid index, irregular contractions in the aganglionic area, irregular mucosal spasm of the colon, filling defects due to fecaloid and no meconium discharge during the first 48 hours of birth with sensitivity and specificity values it is good. Research objective: to assess the diagnostic accuracy of the Hirschsprung colon in loop examination using Alehossein scoring at Dr Saiful Anwar Regional Hospital, East Java province. Research Methods and Samples: analytical observational with a cross sectional approach, using data from 40 patients who underwent a colon in Hirschsprung loop examination and confirmed histopathology examination. Research Results and Discussion: The research results showed that the sensitivity was 96.6%, and the accuracy value was 72.5%, the specificity was 9.1%, the positive predictive value was 73.7%, the negative predictive value was 50%. Conclusion: Colon in loop Hirschsprung technique is able to produce positive results such as histopathology as the gold standard. However, the Colon in Hirschsprung's loop examination cannot predict well in patients who are not suspected of Hirschsprung disease.
