Abstract

Latar Belakang: Karsinoma nasofaring (KNF) menyumbang 73.000 kematian pada tahun 2018, dan Asia Tenggara menunjukkan insiden yang tertinggi. Pada lingkungan mikrotumor terutama FGF, menargetkan sel-sel yang berasal dari mesoderm, seperti fibroblas stroma, perisit, dan sel otot polos pembuluh darah. Meskipun diketahui bahwa FGF memodulasi pericytes untuk angiogenesis dan remodeling vaskular, perannya dalam mendalangi peradangan tumor tidak dipahami dengan baik. Microvessel Density awalnya digunakan dalam kuantifikasi angiogenesis tumor dan penentuan derajat sistem, namun adanya korelasi antara angiogenesis dengan perkembangan tumor, keterkaitan MVD (CD31) dengan kemungkinan metastasis, rekurensi dan angka penyitas serta pemilihan tatalaksana pada keganasan mulai dipertimbangkan. Tujuan: Menganalisis adanya korelasi antara ekspresi FGF-2 dengan MVD terhadap stadium karsinoma nasofaring tipe undifferentiated. Metode: Penelitian ini adalah penelitian observasional analitik dengan desain cross sectional yang melibatkan 24 sampel biopsi jaringan nasofaring dari penderita KNF tipe undifferentiated. Pemeriksaan ekspresi FGF-2 dan CD-31 (marker MVD) menggunakan pewarnaan imunohistokimia yang dinilai oleh ahli Patologi Anatomi. Hasil: FGF-2 dengan ukuran tumor primer tidak memiliki hubungan signifikan dengan nilai P=0,146 (<0,05). Korelasi FGF-2 dengan ukuran kelenjar getah bening memiliki hubungan negatif, signifikan dengan korelasi -0,159 dan P=0,009. Korelasi FGF-2 dengan ukuran metastasis tidak memiliki hubungan signifikan dengan korelasi P=0,899. FGF-2 dengan stadium karsinoma tidak memiliki hubungan signifikan dengan P =0,401. MVD dengan ukuran tumor primer tidak memiliki hubungan signifikan dengan P =0,848, MVD dengan ukuran kelenjar getah bening tidak memiliki hubungan signifikan dengan P= 0,582. MVD dengan metastasis tidak memiliki hubungan signifikan dengan P =0,634. MVD dengan stadium karsinoma tidak memiliki hubungan signifikan dengan P =0,984. FGF-2 dengan MVD tidak memiliki hubungan signifikan dengan P =0,384. Kesimpulan: Ekspresi FGF-2 tidak dipengaruhi oleh ekspresi MVD (CD-31) (p=0,384) pada jaringan nasofaring penderita KNF tipe undifferentiated.

English Abstract

Background: Nasopharyngeal carcinoma (NPC) contributed to 73,000 deaths in 2018, and Southeast Asia has the highest incidence. In the microtumor environment, especially FGF, targets cells derived from the mesoderm, such as stromal fibroblasts, pericytes, and vascular smooth muscle cells. Although it is known that FGF modulates pericytes for angiogenesis and vascular remodeling, its role in orchestrating tumor inflammation is not well understood. Microvessel Density was initially used in the quantification of tumor angiogenesis and the determination of system grades, but the correlation between angiogenesis and tumor development, the association of MVD (CD31) with the possibility of metastasis, recurrence and survival rates and the choice of treatment in malignancy have begun to be considered.Objective: Analyzing the correlation between FGF-2 expression and MVD on the undifferentiated type of nasopharyngeal carcinoma stage. Method: This study was an analytic observational study with a cross-sectional design involving 24 samples of nasopharyngeal tissue biopsies from patients with undifferentiated NPC. Examination of FGF-2 and CD-31 (MVD marker) expression using immunohistochemical staining assessed by an Anatomical Pathologist. Result: FGF-2 with primary tumor size did not have a significant relationship with P = 0.146 (<0.05). The correlation of FGF-2 with lymph node size had a negative, significant correlation with a correlation of -0.159 and P=0.009. The correlation of FGF-2 with metastatic size did not have a significant correlation with P=0.899. FGF-2 with carcinoma stage did not have a significant relationship with P = 0.401. MVD with primary tumor size did not have a significant relationship with P = 0.848, MVD with lymph node size did not have a significant relationship with P = 0.582. MVD with metastases did not have a significant relationship with P = 0.634. MVD with carcinoma stage did not have a significant relationship with P = 0.984. FGF-2 with MVD did not have a significant relationship with P = 0.384. Conclusion: FGF-2 expression was not affected by MVD (CD-31) expression (p=0.384) in nasopharyngeal tissue of patients with undifferentiated NPC
