Abstract
Seiring berkembangnya industri fashion yang semakin pesat khusunya pada bidang sepatu, hal ini menyebabkan permintaan akan produk sepatu semakin meningkat. Perusahaan yang menjadi objek penelitian adalah perusahaan berskala nasional yang bergerak pada bidang produksi sepatu. Produk yang dihasilkan dari perusahaan ini memiliki 13 macam jenis sepatu running yang di desain stylish dan trendy. Dari 13 merk sepatu diambil 3 merk sepatu running yang memiliki cacat tertinggi dan jumlah produksi tertinggi yaitu sepatu dengan merk A, B dan C. Dari hasil wawancara pada pihak perusahaan sepatu merk A, B, dan C diproduksi secara terus-menerus dibandingkan sepatu dengan merk lain. Dari data persentase dan jenis sepatu didapatkan persentase cacat tertinggi dan melebihi batas yang diizinkan oleh perusahaan adalah sepatu dengan merk A yang memiliki persentase cacat sebesar 3,5% dalam periode Januari 2015-Desember 2015. Kegagalan (cacat) yang diizinkan oleh perusahaan adalah sebesar 2% dari jumlah produksi.Tujuan penelitian ini adalah dapat mengetahui bentuk kegagalan apa yang memerlukan prioritas perbaikan, dapat mengetahui penyebab kegagalan dan dapat memberikan usulan perbaikan untuk mengurangi bentuk kegagalan berupa produk cacat. Hasil penelitian ini diharapkan dapat memberikan kontribusi bagi perusahaan dalam mengurangi produk cacat. Penelitian ini menggunakan metode FMEA dan FTA. Metode FMEA bertujuan untuk mengidentifikasi jenis kegagalan pada setiap penyimpangan (cacat) dari spesifikasi yang ditentukan, mengidentifikasi akibat dari kegagalan, mengidentifikasi penyebab kegagalan, mengidentifikasi mode detection dan menetapkan nilai RPN dari hasil wawancara kepada pihak perusahaan. Sedangkan metode FTA bertujuan untuk menganalisis dari atas kebawah, dimana kejadian yang tidak diharapkan yang disebut top event diidentifikasi terlebih dahulu secara mendetail hingga kejadian dasar basic event, untuk merancang FTA digunakan simbol-simbol yang sudah ditentukan. Berdasarkan hasil penelitian didapatkan 3 bentuk kegagalan (cacat) yang memiliki prioritas perbaikan antara lain adalah pengeleman tidak merata, salah alur jahitan dan rusaknya out sole yang diolah dengan fault tree analyisis. Dari 3 bentuk kegagalan diperoleh hasil analisis fault tree tersebut didapatkan 15 penyebab terjadinya kegagalan (cacat) pada sepatu running merk A. dari permasalahan cacatnya sepatu diberikan rekomendasi perbaikan untuk perusahaan yang diharapkan dapat diterapkan dan mewakili semua jenis sepatu di perusahaan, untuk upaya mengurangi bentuk kegagalan dan jumlah cacat.
English Abstract
The demand for shoes are increase along with the rapid development of fashion industry. The products has 13 types of running shoes that are stylish and trendy design. From 13 types of running shoes, there are 3 brand of running shoes selected that has significant defects, which is brand A, B and C. Based on interviews on the shoe company, brand A, B and C are produced continuously compared to the other brands. Percentage data and shoe types shows that brand A has 3,5% of defect percentage in January to December 2015, which is the highest defect percentage and exceed the limit allowed by the company.The defect allowed by the company is 2% of the total production. The purpose of this research is able to know the failure forms that requires improvement priority, able to find out the cause of the failures and able to make improvements to reduce failure forms of product defects. The result of this research are expected to reducing product defects. This research is using FMEA and FTA methods. FMEA aims to identify the type of failure on any deviation (defects) from the prescribed specification, to identify the consequences of failure, to identify the causes of the failure, to identify the detection mode and set the RPN value based on interviews. While FTA method aims to analyze from the top to the bottom where the unexpected occurrence called top events are identified first in detail until the basic event, to design the FTA used the symbols that have been determined. There are 3 forms of failure (defects) which have priority of improvement based on fault tree analysis, such as uneven garnishment, wrong stitching and broken out sole. From 3 forms of failure, 15 causes of failure (defect) occurred in running shoes of brand A. The recommendations are given and expected to be applied and represent all types of running shoes in the company to reduce the failure forms and the number of defects.
